Other priority direction defining development of health care is transition from infrastructure informatization of health care to broad use of digital (digital) technologies in medical practice.

Thanks to programs of upgrade of 2012-2013 communication and computer telecommunication equipment more than in seven thousand state medical organizations is installed, the single protected network of transfer of medical data is organized, federal components of the unified information system are developed.

Already now in 81 regions of the country medical information systems to which the automated jobs of 57 percent of doctors are connected are implemented, 83 regions organized electronic making an appointment with the doctor, 66 – implemented uniform regional dispatching system of ambulance.

For 2016-2018 we are going to implement together with the Ministry of Communications of the Russian Federation the project allowing to connect by the end of 2018 not less than 95 percent of all state medical organizations to a single state information system in the field of health care by implementation of the uniform electronic medical record.

This bill obliging territorial subjects of the Russian Federation to connection to this system is already prepared, underwent public discussion, is approved with all interested federal bodies and in the nearest future will come to the Government.

Citizens of Russia will get access to the protected personal account "My health" on the portal of public services which will provide a broad set of opportunities: making an appointment with the doctor, verification of the policy of compulsory health insurance, attachment to clinic, a call of the doctor on the house, viewing own electronic medical record and obtaining information on a healthy lifestyle and on other important issues.

Not less than 95 percent of the doctors working in territorial subjects of the Russian Federation in the state medical organizations will be connected to a system by the end of 2018. It will allow to pass completely to electronic document management, to refuse maintaining paper documentation by the doctor and by that to release 30 percent of working time on direct communication with the patient.

Besides, succession of treatment of the patient irrespective of the medical organization and introduction of the unified information systems helping to make the decision the doctor will be provided. It is the system of the electronic qualifier which already in active development and will be finished now by the end of the year, including clinical algorithms, the guide, medicinal and other reference books which should be at any time convenient for use by doctors including primary link.

Besides, distance programs of life-long medical education will be provided. For increase in availability of high-quality medical care in each region by the end of 2018 will be implemented: uniform scheduling of service of ambulance that will allow to provide reduction of time of arrival to the patient, telemedicine consultation between specialists of the different medical organizations of a different profile and level – from district hospitals to the leading federal centers.

Besides, the automated system of preferential provision of medicines which not only will improve management of trade reserves of drugs will be implemented, but also will make provision of medicines more address and comfortable for each person. It is in the long term going to connect to the unified information system all medical organizations, including private, within a uniform national health care system and also to extend telemedicine technologies to the relations "the doctor – the patient" with implementation of remote monitoring of the state of health that is especially important for remote regions of the country and for rural areas. Thus, implementation of digital technologies will allow to have a complex positive impact on availability and quality of medical care, a working condition of the doctor and satisfaction of the population.

Inappropriate 700 million on informatization

On June 10, 2016 more than 700 million rubles became known of inappropriate use by the Russian Ministry of Health on informatization.

Department approved the plan of informatization for 2015 with the Ministry of Telecom and Mass Communications, but it was not approved, and in defiance of order of the Government of the Russian Federation No. 365 the ministry directed more than 700 million rubles to informatization of health care.

2013

The Ministry of Health made the IT plan for 2013-2018

On June 20, 2013 the Ministry of Health approved the plan of activity of department for 2013-2018 including including, and indicators on informatization of health care which are going to be reached for this period. Of all purposes specified in this spherethe Deputy Minister of Health Andrey Yurin is appointed responsible for achievement.

In the document five key directions of informatization in which it is necessary to reach specific indicators by years are selected: implementation of the medical electronic record, creation of a common information space for rendering telemedicine services, a possibility of remote monitoring of health of the citizens entering into risk groups, a digital library for doctors and also remote education for medics.

According to plans of the Ministry of Health, by the end of 2013, not less than 26% of medical records of citizens should be submitted in electronic form according to uniform standards. In 2015 this indicator should be 50%, and in 2018 – 95%.

Not less than 95% of federal medical institutions, in 2015 – similar percent of organizations at the level of the interdistrict centers, in 2018 – organizations at the level of municipality should have telemedicine potential in 2013.

Not less than 10% of the citizens entering into group of the increased risk in 2014 should be provided with a possibility of remote monitoring of the state of health. In 2015 this indicator should be 70%, and the absolute scope should be provided in 2017.

As for a digital library for doctors, its volume should grow from 1.25 million pages in 2013 to 6.3 million pages in 2018. The number of remote forms of additional education and advanced training for medics for planning period should increase by 5 times – from 40 to 210.

The Ministry of Health demanded from regions daily IT reports

At the end of July, 2013 the Ministry of Health sent the letter to heads of state agencies in the field of health protection in subjects of Russia with a request to provide daily providing to department data on implementation of regional programs of upgrade of health care regarding implementation of information systems.

The Ministry of Health proves this request by the fact that the management of the President of Russia on application of information technologies and development of electronic democracy holds "a control event (check)" concerning performance of Article 50 of federal law No. 326 "About compulsory health insurance in the Russian Federation" regarding implementation of IT systems.

The specified article defines an order of financing of the regional programs of upgrade of health care including including informatization. Concerning the last the purposes for which budgetary funds are allocated include "implementation of the modern IT systems for transition to policies of compulsory health insurance of a uniform sample including provided with the federal digital application of the universal electronic card, implementation of the telemedicine systems, electronic document management systems and maintaining medical records of patients in electronic form".

Regarding IT the Ministry of Health developed for daily providing data on implementation of regional programs of upgrade and sent to subjects the unified report form including three main points: data on volume of the means provided by the program on IT systems and which are actually spent by the receiver on the volume of the computer equipment provided by the program and actually purchased by the region and also statistical data about the IT systems implemented in the region.

The last, in particular, is detailed to indicators of a possibility of maintaining the electronic medical record of the citizen in the region, record to the doctor in electronic form, exchange of telemedicine data, implementation of EDMS and the uniform register of health workers in medical institutions and also maintaining the electronic passport of medical institution and the passport of health care of the subject.

2012: The Ministry of Health reported on informatization results

On May 24, 2013 within expanded board of the Ministry of Health the head of department Veronika Skvortsova reported to the prime minister Dmitry Medvedev on results of work of the ministry in 2012, including – in the direction of informatization of health care.

According to Skvortsova, in a year in the medical organizations more than 286 thousand units of the computer equipment were delivered, and the quantity of local medical networks grew by 3.5 times.

Service of electronic record to the doctor is implemented in 4 thousand organizations. From the moment of commissioning in December, 2012 about 7 million Russian citizens used it. The Minister of Health recognized that "it is insignificant a little in comparison with negotiability of Russians". Department hopes to complete this fragment of work in 2013.

The possibility of maintaining the electronic medical record, according to Skvortsova, is provided almost in 2.5 thousand medical organizations. also more than 36 million electronic cards are loaded into federal service that makes about 26% of population of the country today.

Among other results of informatization for 2012 Skvortsova noted equipment about 19 thousand cars of sanitary transport and more than 2.5 thousand stations and departments of emergency medical service by satellite navigation of GLONASS and also creation of the electronic directory "Russian Medicine" which total amount of bibliographic records exceeds 500 thousand.

"It is our first steps in the field of informatization of the industry. Together with the Ministry of Communications and expert community we accelerate implementation of all projects", - Veronika Skvortsova said.

According to her, by the end of 2013 department hopes to implement step by step in medical institutions a computer workplace of the doctor with opportunities of remote life-long education, teleconsultations with colleagues, the help in decision making and also reduction of amount of paper document flow.

2011: Concept of Uniform State Health Information System

The Ministry of Health and Social Development approved the new concept of creation of the single state information system of health care (SSISHC) in health care. The document was developed under the leadership of the new Chief information officer of department Vadim Dubinin on the basis of the similar concept prepared by his predecessor Oleg Simakov[1].

The federal DPC of the Ministry of Health which will be created especially for system deployment should form a basis of infrastructure of EIS. It, according to the concept, can be geographically distributed. Regions will be able to use resources of DPC on the IaaS model. On what conditions they will receive this service, in the document does not make a reservation. According to Dubinin, medical institutions will be able at desire to use also own resources, however for this purpose they should configure the gateway for data exchange with the central structures.

Applied components of a system (transactional, managerial and information) in the Ministry of Health subdivide into federal and regional. For example, treat the first, the systems of maintaining the schedule of acceptances of specialists, issues of the directions on the inspections, systems providing carrying out telemedicine consultations and consultations including using mobile devices, the system of accounting and personnel records, various registers, the system of maintaining the electronic medical record and many others.

Creation of EIS is planned in two stages till 2020. At the first stage, in 2011-2012, it is going to develop sections of regional programs of upgrade of health care and different standards, design documentation, to begin to create DPC, to provide medical institutions with computers, etc. For a subsequent period it is supposed to complete standardization efforts, to creation of DPC, to transfer to it the principal centralized EIS components, the federal application software, etc.

Officials expect that creation of EIS will allow to lower indicators of death rate of disability and the complications caused by medical errors, the low level of efficiency, additional costs on treatment out of time diagnosed diseases, to increase the labor potential of the nation due to decrease in temporary and permanent loss of efficiency of the population due to illness, is told in the concept.

2012: Selection of the Ministry of Health from the Ministry of Health and Social Development

The Ministry of Health of the Russian Federation was selected from the structure of the Ministry of Health and Social Development along with Ministry of Labor and Social Security Vladimir Putin's decree of May 21, 2012. The Minister of Health the president appointed Veronika Skvortsova. Rospotrebnadzor was selected from the structure of department and repaired directly to the government.

2010: Golikova - year was one of the heaviest in the social and economic plan

On March 25, 2011 on expanded board in the Government house the Minister of health and social development Tatyana Golikova summed up the results 2010, having called it one of the heaviest in the social and economic plan[2]. She noted that the decisions made in 2010, in particular for support of needy groups of the population despite crisis, were the most considerable for the last 10 years. In particular, valorization of the pension rights of citizens was carried out, social surcharges to pensions which amount was below regional living wages are entered. As a result the average amount of pension exceeded a living wage of the pensioner. At the same time the greatest increases to pensions due to social surcharges are set to receivers of pensions in cases of loss of the supporter and social pensions.

Twice in a year social pensions, at the same time the amount of indexation of benefits to the citizens having children were indexed, and monthly money payments to separate categories of citizens were exceeded by the rate of inflation. In December, 2010 the average amount of pensions was nearly 7600 rub, a work old-age pension of 8200 rub that is 1.45 times higher, than in 2009. Growth of pensions in real terms was 35%. The average amount of the granted pensions was 35.3% of the average accrued salary whereas in 2009 — 27.8%.

According to Golikova, in 2010 it was succeeded to stabilize a situation in the labor sphere. The number of the unemployed decreased from 6.6 million people in the I quarter 2010 to 5.2 million people in the IV quarter 2010, and the registered unemployed — from 2.2 million people to 1.6 million people. In 2010 the real wage grew by 4.2% while the previous year it decreased by 3.5%. Golikova reminded that in 2010 arrears on the salary were reduced by 1.5 times, and the number of the workers working part-time decreased with 725,000 persons to 271,000 people, i.e. by 2.7 times, and requirement of employers for workers grew by the beginning of 2011 by 1.36 times from 724,000 to 982,000 people.

According to Golikova, such results managed to be achieved including thanks to the state measures for employment assistance. According to the Ministry of Health and Social Development, in 2010 the state helped 12.2 million people among whom 456,000 underwent retraining and advanced training, 793,000 took part in the paid social jobs, 85,000 were engaged at temporary works, 285,500 people received a subsidy for opening of own case, 965,500 people were employed on temporary works, 24,800 university graduates were employed.

The special program was necessary for localization of problems on JSC AvtoVAZ and JSC Vyatka and Polyansky Machine-building Plant Molot. Special programs were demanded support of navigators and flight engineers and also the separate program was developed for support of labor market in the North Caucasian Federal District and in monotowns.
