The Integrated Electronic Medical Record (IEMR), crucial element of a system of regional health care, is designed to provide succession when translating the patient between organizations, to increase transparency of a health care system for citizens and to involve patients in care of the health using a personal account "My health". According to the director St. Petersburg Gennady Orlov's MIAC, IEMK is the central link of electronic health care of St. Petersburg. At the moment medical information systems are implemented in 98% of clinics and 80% of hospitals of the city, 70% of doctors have access to jobs, 35% of doctors keep electronic medical records. Already now in electronic form 60% of results of laboratory researches are transferred to IEMK – this indicator is one of the highest in the country.
"We began filling of IEMK with results of laboratory researches – it creates interest of doctors in use of a system. We will continue to fill IEMK with other types of medical documents: directions of doctors, results of diagnostic testings, vypisny epikriza. In parallel with filling of IEMK we conduct work on creation of electronic services for patients. We plan start of a personal account of the patient in August, 2017", – the director noted MIAC of St. Petersburg Gennady Orlov.

Thanks to the solution, the medical institutions and laboratories using different information systems will be able to exchange medical information on the appointed laboratory researches and their results in real time in a uniform format. The attending physician will receive in electronic form result of a research as soon as it is complete in laboratory.
Biological material will go to any of the connected laboratories therefore the attending physician will be able to appoint not only standard, but also specialized researches which could be conducted only in large medical institutions earlier. In the long term commercial laboratories will be able to participate in the project that will allow to expand even more the nomenclature of researches without additional load of the regional budget.
Service is created by joint efforts of the St. Petersburg GBUZ Medical Information and Analytical Center and the Netrika IT company specializing in development and delivery of IT solutions for state bodies.
During creation of service for the first time in Russia the open international standard was applied to health care of FHIR (the most modern version of the industry standard of HL7) at the regional level. FHIR is the open standard created by efforts of the international community that is especially relevant in the conditions of import substitution.
"In the conditions of the reduced budget financing it is extremely important to find cost-efficient methods of upgrade of health care, - Denis Chumakov, the head "Health care" of Netrika company considers. - Using service of data exchange of laboratory researches it was succeeded to lay the technology foundation for effective management of processes of the direction on laboratory diagnostics from the medical organizations of the region. Further this experience will be extended to other projects in the field of health care".
Netrika together with the Russian branch HL7 and the Russian developers FHIR – Health Samurai company, organized training of project participants. As a result already in a pilot project three systems of different developers were connected to service: "Akross. Clinical laboratory", medical information systems "Web Clinic" and "Aura".
"The project is constructed ideologically correctly, - Shvyryov Sergey, the president of HL7 Russia comments. – For ensuring interaction with service it is not necessary to change internal business processes of medical information systems. It is enough to support the interchange format electronic medical data with external systems according to requirements of the standard. It will allow to save as much as possible the systems of different producers which are already working in the region. It is pleasant that the problem of exchange of laboratory data is solved using the international codifier LOINC. It is the reference book which is widely used in the world based on which it is possible to organize data exchange both with domestic, and with foreign medical information systems. Other regions of Russia can quite make use of the accumulated experience and practices at creation of regional solutions in the field of health care".
Adaptation of the international reference book of laboratory tests and services of LOINC for use in Russia became an important stage of project implementation. This task within several months was carried out by specialists of SPb GBUZ MIAC in the commonwealth with scientific medical institutions of St. Petersburg.
"The laboratory service in St. Petersburg is centralized: in the city 14 laboratories which are going to be connected to service of data exchange of laboratory researches work. As a result of implementation of service the term of waiting of result of a research by the patient will decrease, the nomenclature of the researches available in organizations will extend the quantity of errors will decrease, - Gennady Orlov, the director says MIAC. - The organization of the centralized work of laboratories is impossible without uniform reference book of laboratory researches. Our specialists carried out work on transfer and adaptation of the international reference book LOINC for the Russian practice. The new reference book will be used in all city institutions. I am convinced that the St. Petersburg experience will be demanded also in other regions of Russia. The medical information systems developed in St. Petersburg already work in 20 regions of the country, and an integration bus of Netrika company - in eight regions".
