Creation of a monitoring system and support of project office of the priority national Health project.

Since January 1, 2006 the Ministry of Health began implementation of the national Health project – one of four priorities of state policy of Russia. The wide range of the tasks set for the project demanded creation and support of project office PNP and creation of a monitoring system. Specialists of TEKORA-Consulting company were involved to the solution of this question.

The priority National Health project (PNP "Health") is directed to quality improvement of medical care in Russia, including on strengthening of health of the population of the country, decrease in the illness rate, prevention of diseases, improvement of conditions of delivery of health care.

Main objectives of the project:

  • strengthening of health of the population of Russia, decrease in the illness rate, disability, death rate;
  • increase in availability and quality of medical care;
  • strengthening of primary link of health care, creation of conditions for rendering effective medical care at a pre-hospital stage;
  • development of preventive orientation of health care;
  • satisfaction of need of the population for hi-tech medical care.

Within rendering services in creation and support of the Project management system by the Contractor the Project office to which basic functions on monitoring were transferred was organized.

Functions of Project office when implementing the project came down to problem solving in three main areas: methodological providing:

  • identification of problems and broadcast of methodical questions to consultants;
  • support of the developed normative regulatory documents and also the address to consultants in need of their serious processings;
  • tracking of compliance of normative regulatory documents on the project to requirements of the existing regulating documents approved by higher instances;
  • technological support:
  • assistance in problem definition on creation of an automated system, necessary for project management;
  • participation in implementation and support of an automated system;
  • operation of the created automated system and its intermediate versions of working prototypes of a system;
  • administrative providing:
  • administrative support of project implementation, including monitoring process.

The project office functioned in two changes. The first shift worked from 9 a.m. till 6 p.m., a second shift – from 2 p.m. till 10 p.m. that provided the continuity of process of monitoring in all time zones of implementation of Russia.

Object of monitoring is the reporting containing information on the following indicators:

  • The number of the performed money payments to doctors of the general (family) practice, to therapists to district police officers, pediatricians to district police officers and nurses of district doctors of the general (family) practice, therapists of district police officers, pediatricians of district police officers.
  • The number of the trained and retrained doctors of the general (family) practice, therapists of district police officers and pediatricians of district police officers.
  • The number of the municipal out-patient and polyclinic authorities equipped with the diagnostic equipment for the organization of primary health care.
  • Number of cars of emergency medical service, including reanimobiles.
  • The number of the carried-out additional immunizations of the population within the national calendar of inoculations.
  • Indicators of prevention of HIV infections, hepatitises B and C, identification and treatment of sick HIV.
  • Indicator of number of additional medical examinations of the working citizens at the age of 35-55 years.
  • The number of additional medical examinations of the workers occupied at works with harmful and (or) dangerous production factors.
  • Indicator of providing women with medical care in the public and municipal authorities of health care during pregnancy and childbirth (birth certificates).
  • Indicator of number of inspections of newborn children on a galactosemia, an adrenogenital syndrome and mukovistsidoz.
  • Increase in the pay level of work to health workers of the medical and obstetrical centers (MOC), doctors, paramedics and nurses of the emergency medical service (EMS).
  • Number of the organizations of construction of the centers of high medical technologies.
  • Increase in volumes of rendering hi-tech (expensive) medical care (In (d) MT).

When implementing the Project the Contractor constructed the management system and monitoring of the course of implementation of the Priority National Health project allowing to perform:

  • consulting and methodical support of staff of the Ministry of questions of collecting and the analysis of the arriving reporting;
  • regular collecting of the reporting and preparation of consolidated statements and analytical materials;
  • formation of monthly tasks for subordinated departments, the agencies, services, funds for project implementation according to the network diagram PNP;
  • updating of the network diagram of PNP "Health".

The structured system of indicators was developed for monitoring of the Project, units of measure, processes of monitoring of indicators are defined: order and frequency of collecting and bodies, responsible for collection of information. The project office developed document forms for planning of parameters and indicators of implementation of PNP "Health" and also for collection of information on their actual values.

Regular collection of data on indicators of PNP "Health" allows to reveal timely problem areas and a deviation of the actual values from planned that promotes timely development of response measures, warning of the negative phenomena.

The tough regulation of process of data collection which promoted timely providing reports by departments, agencies, services can be referred to strengths of a monitoring system of the Health Project, first. Secondly, the automated system of monitoring of the course of implementation of PNP providing collecting and information analysis on the Project, an information access to all interested persons, formation of reports, necessary for monitoring, and research opinions.
