On June 23, 2016 it became known of the project of integration of a common information space of health care of the capital and MIS GKB No. 1 of N.I. Pirogov. The pilot project was implemented by InterIn company[1].
Project Tasks
The main objectives of the project on informatization of city clinical hospital No. 1 of N.I. Pirogov (GKB No. 1) - business process automation through MIS and creation of the effective tool for the analysis of its activity and acceptance of management decisions by the management. It was required to develop the standard solutions allowing to integrate further MIS of hospitals into a common information space of health care of Moscow, to create a set of methodical documents on the organization of similar projects.
The InterIn Service company became the winner of an open competition in this project.
Project Progress
Works were performed "By InterIn service" in cooperation with Institute of program systems of A.K. Eilamasjana RAHN and Interin Technologies.
When two with small a year ago to me the manager of projects of DIT Alexander Mikheyev came and told - we will make it, I know as - I had serious doubts. Too difficult and big object is represented by our hospital which is located on 22 hectares and includes several territories. It was difficult to believe that the representative of a profession far from medicine knows that it is really necessary for hospital. |
The feature of implementation of MIS in GKB No. 1 - work should be performed about one and a half times quicker while the volume of the implemented functionality is almost twice more. On the course of the project it was necessary to overcome problems with unavailability of infrastructure and total scepticism of personnel.
Network works, included complete reorganization of a LAN architecture of GKB No. 1, were conducted in parallel with project implementation that inevitably led to problems of work of MIS and negatively affected the attitude of the staff of hospital towards her. The contractor had to change an order of works: they were often conducted where technical means, but not were ready where according to the logic of project implementation had to be made.
Works of the first stage of the project carried out step by step. On the first, preparatory, it proceeded about two months, a system was started in work in volume of an accounting circuit which allowed to register patients, to control their movement on medical institution and to create primary statistics on this basis. The accounting circuit is implemented in reception/registry, on posts of medical departments, in department of statistics.
At this stage formed a project team, the headquarters of the project which part representatives of GKB No. 1, IAC DZM, DIT of Moscow and the company contractor were.
As a result of works of the first stage in medical institution accounting of patients is organized and collecting practically of all necessary statistical information is automated.
At the second stage - 3 months, implementation of MIS began in a pilot zone which part the laboratory, drugstore and also the selected departments where there were MIS, necessary for work, a local computer network, computers and specialists, with skills of work for them were.
Then MIS put into operation in the departments which did not enter a pilot zone. Special attention was paid to that users could not only study fluently functionality and be trained to work in MIS, but also get used use fully MIS, to state the notes and wishes on improvement of a system. Representatives of the contractor collected all these data, worked notes and adapted the standard solution to needs of specific users. At the same time the main adaptation of functionality happened at the second stage: until in a pilot zone debugged and finished functionality did not pass to other departments.
In the bulk departments of GKB No. 1 on full mastering of MIS it was required from two to nine months during which intensive support of work of personnel of hospital by employees "InterIn service" was performed.
During implementation of MIS business processes of GKB No. 1 were modified and optimized taking into account use of an information system. For example, for increase in efficiency of process of hospitalization of patients its optimization using a software module "The plan of hospitalization" of MIS "InterIn of Promis" was performed. Flows of the planned patients arriving from a hospital, clinic and the consulting and diagnostic center were considered, cases of the emergency hospitalization are considered.
Project Results
On project implementation more than a year was required. Full-function implementation of MIS in 35 departments is executed, the total number of the unrolled automated jobs exceeded 500, number of the trained users – more than 1 thousand human. For all patients who are in a hospital or serviced on an outpatient basis the electronic clinical records containing the predeterminated data are created. All results of diagnostic and laboratory testings are transferred to an electronic form. All services rendered to patients are fixed in MIS.
According to us, the implementation scope ratio by the time of project implementation was record for implementation projects of MIS in one medical organization. In the project in 13 months MIS was set, works on setup and adaptation are performed, substitution of the third-party systems and data integration is made and also the functionality of MIS "InterIn of Promis" in more than 500 workplaces at which more than 1 thousand users work is implemented. |
Thanks to joint hard and harmonious work of personnel of hospital and the staff of InterIn company, to high professionalism of project managers, we received the tool for management of activity of hospital. And though works on development of a system are continued, we already see its potential for management of medical and diagnostic process, quality and cost value of treatment. |
Implementation of MIS significantly simplifies, and, above all accelerates many processes connected with medical and diagnostic work. For example, the doctor can see analysis results at once as soon as the laboratory executed them. Or the advising specialist can obtain the preliminary information before survey of the patient. Significantly the efficiency of use of the expensive medical equipment due to planning of loading, whether it be the specific device MRT or the operating room increases. Repeatedly load of employees of IT department of hospital increased. Department more than 1 thousand active users working at the large territory in the body, between which distance more than a kilometer ensure functioning. Much the useful value of the computer equipment grew.
Implementation of the second queue is going to be carried out using the standard solutions and documents defining methodology of informatization of hospitals developed during the project. Considering high importance of inclusion of organizations of stationary type in information circuit of EMIAS, high resource intensity of these works, completion of complex informatization of GKB No. 1 together with the developed methodological, technical and organizational solutions will fully allow to approve architecture of inclusion of MIS for hospitals in structure of EMIAS, to optimize requirements to the standard solution MIS for hospitals and to integration mechanisms and, finally, to reduce costs for informatization of hospitals of the city.
We hope that DIT will not stop on a half of a way and will help us to complete begun – automation of all departments of hospital. We are ready to share already got experience and to acquire new regarding solving of tasks of increase in efficiency of hospital, its integration into EMIAS. |
