K-MIS: Central register of patients

TsRP is the specialized solution allowing to accumulate and manage the consolidated regional database in which the information about the patients serviced by a health care system of the subject of the Russian Federation is stored. At the same time in the database can be registered as the patients already once addressing to the medical organizations and potential patients of MO who for various reasons can ask for medical care. TsRP in completely automatic mode performs formation of different thematic registers and lists of the population, for example – the register of the insured population, the register of the assigned population, regional registers of exempts, disabled people, etc. At design of TsRP the principles of work provided in so-called Master Patient Index (MPI) - the specialized systems intended for collecting and storage of demographic information on patients were considered.

2017: Central register of patients" version 2.0

TsRP of version 2.x is absolutely new solution written completely from scratch. The main differences of version 2.x from the previous versions 1.x are:

• Full compliance to requirements of import substitution. The new version is based on open software platforms – the nginx Web server, the Python application server and the PostgreSQL database management system. The solution does not use the licensed or closed protocols and general-system software of foreign production, is completely Russian software product. The previous version of TsRP 1.x was constructed based on the closed software platform of IBM Notes/Domino which use in new products in K-MIS decided to refuse.
• Lack of software restrictions on the DB size. TsRP of version 1.x owing to restrictions of a development environment had the maximum limit of the database of 64 GB. that could affect in large regional projects. The new version of TsRP 2.x has no software restrictions, its database can contain millions of records on different objects, and the sizes to exceed terabyte values.
• The increased performance. The architectural concepts and development tools applied during creation of a system provide unknown system performance, even at a high load and the DB big sizes in several million records. Moreover, the centralized method of data storage and application of the REST Full API protocol allow to minimize load of communication channels and to ensure stable system operation in the mode of its integration with an unlimited number of the medical organizations even in those projects where there are problem communication channels in terms of capacity.

Comparison of some utilization properties of TsRP 1.x and TsRP 2.x is given on diagrams below.

• Full support OPEN SOURCE, any computers as the client workstation. A system is constructed for 100% use of Open Source, including operating systems of servers and workstations of users. Access to a system and a full-fledged work with it are supported for computers with any OS where it is possible to start the standard Internet browser – Windows Linux Mac os, and even work from tablet computers under management Apple iOS Android Windows Mobile. System operation does not require acquisition of the proprietary software.
• Open API for connection of any MIS. The previous version of a system supported full integration via the special integration gateway KIIS.RIR, but in general with it was integrated and directly only MIS of production of our company interacted. In the new version completely independent integration API allowing to connect any adjacent medical information systems is supported. For work of TsRP MIS or other software solutions of our company are not required, it is completely autonomous and independent software product therefore does not create a conflict of interest for developers other solutions.
• Completely autonomous independent normative reference information (NRI). For work of TsRP 2.0 the high-performance NSI of the new version which is completely integrated into the solution, but at the same time constructed in strict accordance with a format and architecture of Federal NSI is used. System administrators can make quickly and conveniently changes to reference books of TsRP 2.0 which will be used by all MIS of the region.
• Built-in analytical system. Own analytical system which allows to prepare all necessary reporting – to build statistic reports with flexible data filtering is built in TsRP 2.0, to create different diagrams, charts, etc. At the same time the speed of work of these functions is significantly higher, than in the applied 1st version of the Statistics program. Now administrators and users of a system can receive all necessary reporting directly on the basis of the stored DB without additional transfer it from an accounting part in specialized applications.
• Automatic formation of registers. In the new version the technology of automatic formation of registers, including with adding of the necessary new registers and registers by settings is developed. Such approach does not require long development and release of new versions of TsRP if Customers need to add the new register on the basis of TsRP DB. All this becomes group of technical maintenance independently by setup corresponding system reference books.
