The Russian surgeons first-ever performed operation on installation of a titanic implant of front department of a basin

On April 18, 2019, after removal of a massive cancer tumor, the cross-disciplinary team of surgeons of the Sechenovsky university carried out the first in Russia and in world practice operation on installation of a titanic implant of front department of a basin. Surgeons – the director of Clinic of coloproctology and low-invasive surgery Pyotr Tsarkov and the director of Clinic of traumatology, orthopedics and pathology of joints Alexey Lychagin took part in transaction.

Cancer of the anal channel was initially diagnosed for the patient (52 years), and operation on the residence in the Crimea is successfully performed. Further there was a recurrence which demanded removal of a rectum, a uterus, a bladder, a front wall of a small pelvis – an evistseration – the big transaction atypical for cancer of the anal channel.

Under the influence of the combined treatment methods cancer tumors change, and the recurrence arose in a bladder neck with germination in a pubic bone. The tumor quickly progressed, we looked for the solution, – professor Pyotr Tsarkov performing operation of an evistseration of bodies of a small pelvis explained. – The only exit in this life-threatening situation – an extensive resection, the operation on removal of a tumor destroying integrity of a pelvic ring.

Surgeons of Clinic of coloproctology and low-invasive surgery under the leadership of Pyotr Tsarkov decided on transaction, previously having approved it with traumatologists that afterwards, having applied the innovative technologies to solve a problem of destruction of a pelvic ring. It was impossible to make it along with operation on removal of a tumor: in such complex cases the volume of a resection is defined in the course of transaction, and the size of a prosthesis is defined after transaction.

The decision was made that coloproctologists do the part of work – an evistseration of bodies of a small pelvis with a resection of a pubic bone into which the tumor, and pictures after transaction, and traumatologists the – 3D - reconstruction, precise measurement of the volume necessary for substitution of a remote bone tissue a titanic prosthesis grew. Volume turned out so big that use of normal technologies, for example, of an osteosynthesis, in this case is inefficient. The original solution was necessary, and traumatologists found it: application 3D - technologies.

With each patient we work for recovery, and, passing this, sometimes very long, a way, we do not allow it to doubt it. The personalized approach is important in all fields of medicine as the object of the labor of the doctor is health and needs of the patient, – the traumatologist-orthopedist Alexey Lychagin is convinced. – In a case with our patient we will recover the continuity of a pelvic ring, using cross-disciplinary approach and opportunities 3D - technologies.

Team of surgeons of the Sechenovsky university, making break in treatment of the heaviest patients and opening the new directions in orthopedics, for the first time successfully performed operation on reconstruction of a pelvic ring by means of the titanic implant printed on the 3D-printer. Specialists are sure that several weeks later the patient will be able to move fully, thereby, significantly having improved quality of the life.
