2023: Digitalization of healthcare of the Krasnoyarsk Territory

On March 9, 2023, TechLAB announced that the integration of two regional information systems RVIMIS and RLIS, necessary for the development of digital health care in the region, has been completed in the Krasnoyarsk Territory. The developers and integrators of the systems were the IT companies TechLAB and KOMTEK. As a result of the integration, laboratory data, without which it would be impossible to form a full-fledged clinical picture of the patient, became available to doctors and health care organizers at the regional and federal levels.

Digitalization of healthcare of the Krasnoyarsk Territory

According to the company, laboratory research is an integral attribute of almost any diagnosis of the disease. As prescribed by the doctor, the patient may need to donate biological material (blood, urine, feces, saliva , etc.) for general clinical or biochemical analysis, analysis for hormones, etc. In the same row cytological and histological studies are located when cells or tissues of the body are studied in the laboratory. It is these studies that are indispensable in the diagnosis of malignant neoplasms: their results are the basis for making a diagnosis and choosing the right therapy protocol.

However, just conducting a study for high-quality diagnostics is no longer enough: it is necessary that its results are automatically converted into a structured digital form, which in the same automatic mode became available to the attending physician. In addition, it is important that these data are taken into account in the preparation of statistical reports and in the calculation of consumable reagents both in a specific medical organization and in the region.

Researches

To achieve these goals, the Krasnoyarsk Territory is served by the RLIS - Regional Laboratory Information System, which was developed by the KOMTEK IT company, which automates medical institutions. In order for the laboratory data aggregated in this system to be used to make management and analytical decisions at the level of the Krasnoyarsk Territory, the RLIS must be integrated with the Regional Vertically Integrated Medical Information System (RVIMIS), developed by the St. Petersburg IT company "TechLAB" (St. Petersburg), which develops digital health services. Both companies announced the completion of such integration.

We were engaged in the creation of RVIMIS in the Krasnoyarsk Territory in 2021, and in 2022, as part of the development of this system, we organized information interaction with the RLIS. Thanks to this, RVIMIS gains access to laboratory data, in particular histological studies. Without such data, you cannot get a full-fledged clinical picture of the disease, which is needed by both the doctor and the health care organizer to make the right decisions.

Integration of two RVIMIS and RPGA regional information systems

Interaction between RVIMIS systems from TECHLAB and RLIS from KOMTEK is arranged as follows:

  • first, the results of the laboratory test are recorded in the RPGA;
  • then the signal with the referral or medical report data is sent to the RVIMIS, where the necessary processing takes place, in particular, the format conversion and conversion of regulatory reference information, after which it is added to the patient's card, enriching the clinical data already available in the system;
  • Further, the RVIMIS forms a document in the format of a SAMD (structured electronic medical document) on the basis of this information;
  • finally, the results of laboratory tests fall into the federal VIMIS service in the format necessary for this system.

In other words, the integration between RPGA and RVIMIS is based on the principle of notification of the appearance of data, for which the RVIMIS developers needed to implement SPI (Service Provider Interface) provided by the RPGA developers. Interaction between systems is carried out using the HTTP protocol.

According to the expert "TechLAB," the information interaction of the two systems is important for the correct operation of health services at other levels, including the federal, and a special role is played by the format in which medical data are transferred to the federal services of VIMIS. Thus, medical information received from the RLIS must be transformed into the SAMD format before it is sent to the federal VIMIS services.
