Roche cobas 4800 is building block system for PTsR-diagnostics of in vitro. It is intended for identification and determination of concentration of nucleinic acids in clinical samples by the PTsR method in real time at diagnostics and screening researches.

PTsR method

PTsR ("polymerase chain reaction") is the method of laboratory diagnostics directed to identification of causative agents of infectious diseases. The analysis allows to detect presence of genetic material of infectious activators. PTsR in gynecology and in urology is widely applied to detection of the hidden and hardly diagnosed infections. Including PTsR and on HIV is executed.[1]

2020: The new test for a coronavirus is approved

In the middle of March, 2020 the Food and Drug Administration (FDA) approved the new test for a coronavirus which will allow to accelerate testing of patients ten times.

The new test is carried out on the Cobas 6800/8800 systems of Roche company. System 8800 is capable to test 4128 patients a day, version 6800 can test up to 1440 patients. The Roche Cobas systems started in 2014 are widely available worldwide: 695 tools from tool series 6800 and 132 from series 8800 are used.

The U.S. Government and the most part of Europe is criticized for sluggishness and inefficiency of measures of prevention which allowed a virus to extend across the huge territory. Testing — one of the main measures of prevention of distribution of Covid-19 as allows health workers to reveal infected and to place them in a quarantine even if they almost have no symptoms. It will allow to reduce a total quantity of infected and to win time for drug manufacturers and developers of vaccine.

It is the third and first commercially available test which got approval of regulatory bodies of the USA in response to emergency. In February the diagnostic tools provided to CDC and Department of health care of the State of New York were already approved. Nevertheless, the Cobas 8800 system can test patients about 10 times faster, than the former tests executed on MAGNA Pure 24 and LightCycler 480 devices. However, these diagnostic systems are more often used in laboratories and hospitals worldwide therefore will continue to play a key role in testing of people, especially outside the USA.

Cobas 6800/8800 devices allow to receive result of a research within four hours. The Roche company is ready to provide millions of tests for systems every month and "reached limit of the production capacities", says the company.[2]

2019

Components

For January, 2019 are composition of cobas 4800:

  • the device for preanalitichesky preparation of PTsR-analyses of cobas x 480 is the automatic multichannel digging-out device, is used for selection, cleaning and preparation of nucleinic acids for the subsequent testing by the PTsR method (simultaneous sample preparation up to 96 samples);
  • The cobas z 480 PTsR-analyzer is represented by a termotsikler from the detection integrated by a system in real time (amplification and detection);
  • the system unit contains the software of cobas 4800 software (management ^bas x 480 and ^bas z 480) and the editor of orders of cobas 4800 Work Order Editor.

A system can be connected to LIS (laboratory information system) for an automatic task of orders for laboratory tests.

Descriptions of an operation principle

The operation principle of analyzers is based on use 5' nukleazny technology using marked by two fluorescent dyes of probes which fluorescent signal is extinguished at stay in a basic status. During polymerase chain reaction direct radiation from an excitation source to samples. The induced fluorescence is detected by a recording system and displayed on the monitor screen in a schedule type. As a source of excitation the LED is used. Using a fluorimetrichesky system fluorescence of each sample is measured during thermocycling. When using different combinations of filters a system can read out four wavelengths. The photodetector is CCD the camera. As of January, 2019, using a system tests for HPV (papillomavirus the person), CT (chlamydia), NG (gonokokki) can be carried out.[3]
