Regional information resource of KIIS (KIIS.RIR)

The "End-to-end Medical Information Systems" company announced release of a new solution – the integration platform for creation of regional fragments of the state information system in the field of health care.

As you know, a main objective of creation of a single state information system of health care (GIS-Zdrav) is ensuring effective information support of process of management of a system of medical care and also process of delivery of health care. General provisions of GIS-Zdrav, structure, requirements and functionality are developed and submitted to Concepts, approved as the order of the Ministry of Health and Social Development No. 364 of April 28, 2011. The idea of the concept consists in creation of the general state accounting system and processing of the personified information on the provided medical care. The main participants of a system are regional governing bodies of health care, territorial departments of Federal Compulsory Health Insurance Fund and the treatment and prevention facilities (TPF) and also citizens and educational institutions.

As you know, GIS-Zdrav will consist of two principal components:

• The federal data processing center (DPC) for functioning of the federal information resource (FIR) of a system. FIR will dry requirements of the Ministry of Health and Social Development and other state authorities at the federal level. On it it is going to obtain in a foreground mode all necessary analytical information on system operation of health care of the whole country in general.
• The regional fragment of GIS-Zdrav, consists, in turn, of two modules:
  • The Medical Information Systems (MIS) in the treatment and prevention facilities (TPF). A task of MIS – maintaining by personnel health facilities of medical documentation in electronic form for solving of tasks of the health facilities (quality improvement of medical documentation, cutting of costs for processing and information storage, increase in availability of medical data) and also transfer on the regional level of necessary primary data about the medical care provided in health facilities.
  • Regional Information Resource (RIR). Tasks of RIR – collecting from health facilities of primary medical information in electronic a type, its depersonifitsirovaniye, formation of the necessary analytical reporting (the state forms of statistical reporting, financial statements, information on resources of health facilities, the personnel, incidence, etc.) for the purpose of management of a regional health care system. The second important task of RIR is transfer of the consolidated work indicators to FIR.

Traditionally the K-MIS company specialized in projects of automation of health facilities, without being engaged in development and deployment of solutions of "top level" (the company involved third-party developments to regional projects). However recently, studying a number of regional projects and meeting requirements of the market, the company faced the real need to be engaged not only automation of directly medical institutions, but also to solve a problem of consolidation of health facilities in a region common information space for the purpose of creation corresponding regional information resources (RIROV).

For the solution of this task the K-MIS company made the decision to develop and put on the market the special software (S) which turned out the name "Regional Information Resource of KIIS" (further – KIIS.RIR).

The purpose of creation of KIIS.RIR is consolidation of the data, separate applications, components and systems created and used by regional governing bodies of health care in the general information space.

Tasks of KIIS.RIR are:

• Integration of the separate applications which are components of the regional information resource (RIR) in the general interface space (in the general user interface).
• Information exchange between the RIR components.
• Integration of RIR with the federal information resource (FIR).
• Implementation of separate tasks and automation of the separate areas of work provided by regulating documents as the RIR functions.

The impressive portfolio of the separate solutions for the regional level of use which are saved up for the last few years by K-MIS company became premises of release of a new product. Earlier these solutions were proposed as additional paid options by delivery of a flagship product of the company – "An end-to-end medical information system". However development of projects of informatization of health care and transformation of the market of solutions in the field of medicine became so notable that there was a need to consolidate all these separate solutions in a single regional information system.

When developing KIIS.RIR one of the major principles is the principle of integration of the existing separate components (both the production of K-MIS company, and the third-party solutions created and implemented in different regions) in KIIS.RIR common information space. It is no secret that now in the majority of regions there are already separate components which are successfully developed (or purchased in public market of software) and implemented in practical activities of the region. The K-MIS company considers that if any separate element of a regional segment is already implemented, or regional governing bodies of health care already selected for themselves certain products for this task, then should offer KIIS.RIR not replacement of these modules, but their effective integration. Moreover, at present actually there are no uniform requirements and detailed specifications to the RIR separate components – in many regions and the MIS different developers have own views and understanding as this or that segment should be suited and work.

Considering the aforesaid arguments, when developing architecture of KIIS approach to free modularity of components of KIIS.RIR was provided. Actually, KIIS.RIR is certain "cover" into which it is possible to integrate any third-party information modules. Only a small part of components of KIIS.RIR is built-in and is always delivered with this solution. The majority of components of a system are free options which can be or in general are excluded from structure of KIIS.RIR in the specific regional project, or replaced with some specific solutions selected by the customer. For this purpose as a part of KIIS.RIR there is special means allowing the IT specialist to integrate any any solution in a system.

The second important feature of KIIS.RIR is the principle of partnership. The K-MIS company is going to develop and support, generally only a general-system part of KIIS.RIR and a number of applied modules (NSI, an informational portal, exchange of medical documents in HL7 format, etc.). The main and major part of KIIS.RIR will be made by the partner solutions developed by the third-party companies and which proved as reliable and software products demanded by the market. For this purpose the K-MIS company conducts different negotiations on filling of KIIS.RIR partner to solutions and integration into KIIS.RIR of other medical information systems.

Now the architecture of KIIS.RIR includes 3 components: general-system services, application solutions KIIS and third-party specialized solutions for separate tasks.

The following services, services and opportunities are a part of general-system components of KIIS.RIR:

• IBM Lotus Domino with a possibility of group work of users and implementation of the most various applications, including CRM, systems electronic document management (EDMS), etc.
• The mail server with powerful and one of the implementations of the mail system, most popular in the world, supporting work both in the thick client (IBM Lotus Notes), and in the thin client (all major versions of browsers are supported, including Microsoft IE Firefox Safari Opera , etc.) and mobile devices (Apple iPhone Google Android RIM BlackBerry Apple iPad etc.).
• The Web server with a reliable security system and development of any websites and web applications.
• Directory of users, including support of LDAP
• Powerful security system on the basis of role access, enciphering and management of lists of access control. The safety system of IBM Lotus is one of the most protected in the world.

• Support of web services for integration of the most any information systems

The applied KIIS.RIR component includes the following applications:

• Federal NSI. The reference book from federal NSI, is necessary for KIIS in health facilities, is created on the basis of the order of Federal Compulsory Health Insurance Fund No. 79 (and amendments to it). Updating of the reference book is performed by KIIS developers.
• Regional NSI. Reference books specific to this region (health facilities, insurance companies, etc.). It is necessary for work of the personified system of accounting of medical care and to settlement on compulsory health insurance. It is created on the basis of the order of Federal Compulsory Health Insurance Fund No. 79 (and amendments to it). It is updated by specialists of RIR (the customer's specialists).
• General-system reference books of KIIS. Include KLADR, radar station, MKB-10, the reference book of medico-economic standards, etc. Are used during the work of KIIS in health facilities. The centralized maintaining these reference books allows to simplify and cut down significantly expenses on administration of the distributed installations of KIIS in health facilities.
• Global Reference Book (GRB). Special software, allowing to extend any KIIS settings in health facilities it is centralized from RIR, also centralized keeping these reference books, performing their automatic update (updating).
• Central register of insured. A special DB for the centralized storage of the register of insured. Contains software for automatic update of the central register from external DB and data sources.
• Central registers of patients. Special installation of the KIIS some components allows to maintain on the KMIS server common regional registers of patients (for example, the register of patients with diabetes, the general DB of recipes on DLO in the region and many other things).
• Central archives of medical images. Integration into KIIS.RIR of archives of the medical images accumulated and updates in health facilities, allows to aggregate a DB of medical images of the region.
• Regional informational portal concerning health care. A possibility of creation of the website of a health care system of the region, integration into the portal of record to the doctor via the Internet, the full-fledged web client for management of the website, weight additional opportunities, including maintaining library of documents, databases of the legislation, discussion forums, wiki-libraries, etc.
• The portal for record to the doctor via the Internet. The regional portal for record to the doctor via the Internet includes. Supports connection of information booths. It is going to add support SIEI and integration into portals of public services.
• Information Exchange System (IES). Allows to receive at the request of one health facilities the medical record or the separate document from other health facilities or to transfer it. As the transport protocol for such transfer it is possible to use the corporate protected network, or to perform export/import of necessary documents in electronic form on the USB stick (removable medium). For example, SIO allows at the direction of the patient from a hospital to send to clinic following the patient it vypisny the epicrisis in electronic form. Supports as data exchange in the closed format of electronic documents KIIS (on the basis of DXL), and in HL7 format.
• Death rate monitoring system. Allows the death certificate is centralized to keep account medical, to prepare statistical reporting on death rate.
• Telemedicine KMIS system. The telemedicine KMIS system based on the solution of IBM Lotus Sametime allows to connect any quantity of jobs by the reliable system of a video conferencing, to carry out video consultations, consultation, distance learning. There are functions of remote control a desktop, display of a presence status of users, instant messaging. Completely cross-platform solution supporting a broad spectrum of telecommunication equipment, video cameras and also integration into Skype.

Updating of fall of 2012

Implementation of the completions necessary for successful project implementation of creation of regional segments of the Single state information system in the field of health care (Uniform State Health Information System) became the main area of work of the company by preparation of release of fall of 2012. The new KIIS.RIR version includes the following new features and completions:

• the new application "Passport of the Medical Organization" which allows to keep account of information on all of medical institution of the region. A system represents the instrument of accounting of the general management information about the medical organizations, their departmental and territorial accessory, an organization and legal structure, the available licenses, the used information systems, etc. A system provides a possibility of obtaining the analytical information characterizing a status of resources of health facilities of the region necessary for accomplishment of problems of monitoring, the analysis and conformity assessment of material and technical resources of the medical organizations. The structure of fields and credentials corresponds to GOST 52978-2008 "Structure of data on treatment and prevention facility for Electronic Data Interchange";
• the new application "Register of Medical and Pharmaceutical Personnel" which allows to keep account about a personnel status of a regional health care system. A system represents the instrument of monitoring of staffing and the personnel needs for a part of health and pharmaceutical workers, planning of volumes and structures of preparation, retraining and advanced training of the medical and pharmaceutical personnel, control of timely training, also coordination of level of its salary with quality and volume of the provided medical care allows to execute assessment of qualification of medical and pharmaceutical personnel. A system provides receiving by the interested bodies and the organizations of the analytical information including allowing to perform planning, monitoring, assessment and control of structure and volume of medical and pharmaceutical personnel;
• the new application "Register of the Medical Machinery and Equipment" representing the instrument of monitoring of equipment of the medical organizations by necessary material resources for the purpose of the analysis of compliance of material security of the organizations to standards of equipment, optimization of purchases of supplies and component parts, monitoring of load of the high-tech medical equipment and information analysis about a status, operation and use of program technical means;
• the improved integration of Karelian MIS and KIIS.RIR. In particular, the special means allowing to automate filling of data in Passport of MO and the corresponding accounting registers on the basis of information which is already entered to the reference book KMIS of health facilities is built in KIIS.RIR. The new setup allowing to use a part of credentials from KIIS.RIR during the work with reference books of KIIS in health facilities is built in KIIS now;
• integration of KIIS.RIR and "Telemedicine KMIS system". Now users of KIIS.RIR can use also effectively various opportunities of telemedicine, as well as users of Karelian MIS in health facilities. In particular, instant messaging tools, videokonferentsvsyaz, exchange of medical images, virtual meetings and consultations, presence indication, personal contact lists and a set of other functions are available;
• new opportunities in NSI. The range of reference books in Federal NSI and Regional NSI which allow to conduct is significantly expanded it is centralized on all to the region one regular set of the normative reference information necessary for work of accounting services, such as Passport of MO and the corresponding registers;
• the improved and added documentation. The structure of KIIS.RIR includes a big set of documents called by "The guide to creation of a regional fragment" where all stages of creation of a regional Uniform State Health Information System segment, including recommendations about creation and effective configuring of DPC, the installation instruction and setup of the general-system and application software, instruction for the KMIS setup in health facilities for work on the single regional information network, samples of regulating documents, such as orders and orders on the region, acts of execution of works, "The passport of the project", etc. are step by step explained. All submitted documentation is supplied with necessary explanations, examples and links to the additional specifying tekhnicheska. documentation. Using this set of documents partners and customers of K-MIS company I can significantly simplify and increase efficiency of execution of works on implementation of necessary software;
• the new version "The central register insured", containing improvements in accounting of exempts, the improved interface, support of accounting of exempts on 7 high cost nozologiya (7VZN), new opportunities for automatic distribution and updating of information about the insured on all health facilities of the region and some other innovations;
• the built-in "Electronic registry 2.3", with expanded functions of maintaining a personal account of the patient, maintaining the personal electronic medical record (PEMR), etc.
