Surgeons of Clinic of faculty surgery of N.N. Burdenko Sechenovskogo of the University for the first time performed unique operation on the complexity to the patient with cancer of a gullet. By means of the robotichesky surgical da Vinci system they deleted the gullet affected with a tumor and in one step completed a reconstructive phase — created a new gullet from a stomach, reported TAdviser on September 10, 2018 at the Sechenovsky University.
According to representatives of the university, operations at cancer of a gullet are considered as one of the most difficult surgical interventions. Surgeons work in three anatomic zones at once — in a chest and abdominal cavity and also at a neck. Traditional transactions are traumatic, require big cuts to provide optimal surgical access. As a result the wound surface increases, the surgical injury and a stress answer on surgery increases. It can lead to the expressed pain syndrome, risk of postoperative complications and increase in duration of treatment of the patient in a hospital.
The operation performed by surgeons the robot-assistirovannaya took place in the planned mode and lasted 4.5 hours. On a response of specialists of Clinic of faculty surgery of Burdenko, the da Vinci robot provides the improved stable visualization in difficult and narrow anatomic zones, allows to carry out accurate manipulations, without injuring important bodies and vessels (heart, a trachea and bronchial tubes, trunk vessels) and also minimizes blood loss and risk of other intraoperative complications.
For the first time at this transaction type we carried out marking of a tumor dye Indo-cyanine to green (ICG) that provided the fluorescence mode for intraoperative navigation of da Vinci, accurate visualization of a tumor and regionarny lymph nodes. Also using fluorescent ICG visualization control of adequate blood supply of the found transplant from big curvature of a stomach (a new gullet) and the created anastomoz was exercised — Fedor Vetshev, the surgeon-oncologist of Clinic of faculty surgery of N.N. The uniqueness of this transaction consisted that it was carried out to the patient with the limited sklerodermiya — CREST syndrome (multiple kaltsinata, Reynaud's disease, ezofagit, a sklerodaktiliya, teleangioektaziya) — a rare chronic polysystem disease, noted at the Sechenovsky University.
