2020: Application in cancer therapy of a palatal tonsil

On September 2, 2020 it became known that at Institute of surgical correction and recovery of V.D. Reshetov gave chance of rescue to the patient with a recurrence of cancer of palatal tonsil.

During transaction the method of an occipital rag was applied.

The majority of oncological operations for malignant defeat in area of the head and neck lead to creation of the difficult extensive combined defect. Recovery of function, volume and for faster rehabilitation of the patient, requires elimination of this defect created during transaction.

The volume, the sizes, degree of prevalence and localization of loss of fabrics are contributing for the choice of a rag. Reconstruction of the combined defect is a difficult task and is solved by means of individual approach to each case. At the same time closing of surgical defect with local fabrics if it is possible, is more preferred choice of plastics.

The occipital (back and cervical) rag is the representative of a difficult skin and muscular segmented rag. So the rag has the wide basis and krovosnabzhatsya from several arteries (an occipital, back ear artery). This method of reconstruction is used only at serious defects. At the patients who received treatment concerning malignant new growths of the head and neck, this method of reconstruction allows to receive the healthy, well krovosnabzhayemy fabrics capable to get accustomed in a wound bed.

The occipital rag well proved in onkoplastichesky surgery as it is capable to substitute big deficit of fabrics that expands indications to transactions at the patients who were earlier considered as inoperable and nekurabelny.

This method was applied to treatment of the patient M. with progressing of the main disease (cancer of the left palatal tonsil of pT2N3M0. IVB of the article) – a metastasis of planocellular cancer in soft fabrics of a lateral surface of a neck at the left. Earlier to the patient surgical treatment in volume a tonzillektomiya, a neck limfadenektomiya was executed at the left, also the chemotherapy is carried out beam. The volume of defeat of soft fabrics, proximity of arrangement of a tumoral node to an internal carotid (2 mm from VSA according to KT) and resistance of a tumor to beam and chemotherapy speak about need of radical surgical treatment.

To the patient broad coretraction of a tumor of soft fabrics of a lateral surface of a neck was executed. As a result of surgical treatment vital structures of a neck were bared. For the purpose of plastic elimination of surgical defect and the shelter of an internal carotid the occipital rag was applied.
