Objectives
Not less than 99% of the state medical organizations in Russia will have to implement medical information systems and to integrate them with regional information systems and components of a single state information system in the field of health care (Uniform State Health Information System).
Not less than 99% of jobs of medics in the organizations providing primary health care should be retrofitted by a computer hardware, the automated jobs connected to medical information systems and electronic signatures.
The Government of the Russian Federation intends to carry out both tasks by November 10, 2025, said in the passport of the priority project "Improvement of Processes of the Organization of Medical Care on the basis of Information Technology Implementation" which was approved on November 8, 2016 by the prime minister Dmitry Medvedev (cm the document entirely).
According to the document, in all 85 territorial subjects of the Russian Federation in 2025 processes of delivery of health care using telemedicine technologies will be organized.
In 2018 100% of the Russian citizens insured in the system of compulsory health insurance will have to have electronic medical cards.
The number of users of the electronic medical services available in a personal account "My health" (it will be started on the portal of state services in the first quarter 2017), in 2025 should make 48 million people.
In a personal account it is going to organize not less than 10 electronic services for providing to citizens.
First of all, there will be an opportunity to make an appointment with the doctor, to call the doctor on the house, to receive data on the provided medical care from the electronic medical record, data on the policy of compulsory health insurance and medical insurance company, data on attachment to the medical organization.
The following queue of services are electronic medical documents, data on the rendered medical services and their costs, execution of the policy of compulsory health insurance, assessment of satisfaction with quality of work of the medical organizations (including development of the system of quality control of work of the medical organization by means of this service), record on planned medical examinations.
Project Implementation
2018: Selection of 2 billion rubles on implementation of MIS in regions
The head Governments of the Russian Federation Dmitry Medvedev signed on April 12, 2018 order No. 659-r about assignment in the amount of 2 billion rubles from a reserve fund of the Government on implementation of medical information systems (MIES) in the organizations health care providing primary health care. According to the document, budgetary appropriations will be sent to 75 regions Russia and distributed according to the requests which arrived from them.
We for the first time select money that in each clinic, each hospital or other medical institution where primary medical care is provided, it was possible to pass to electronic document management. The order is prepared by the Russian Ministry of Health for the purpose of implementation of the priority project "Electronic Health Care". According to the priority project, for the purpose of increase in efficiency of rendering medical aid to citizens at the expense of information technology implementation the medical organizations should provide maintaining medical documentation in electronic form and also integration with a single state information system in the field of health care (Uniform State Health Information System) for implementation of electronic services in a personal account of the patient "My health" on the uniform portal of state services.

It is supposed that the allocated funds will allow to provide achievement of the following results:
- use by the MIS medical organizations, their automated interaction with GIS in the field of health care of the subjects of the Russian Federation and Uniform State Health Information System;
- maintaining in the medical organizations of schedules of acceptance of doctors in electronic form;
- remote record of citizens on appointment using the uniform portal of state services;
- electronic medical document management, including maintaining electronic medical records of patients, exchange of documentation in the form of electronic documents between the medical organizations;
- formation of accounts for the rendered medical aid for payment to territorial funds of compulsory health insurance and the automated information exchange with information systems of these funds.
Data on medical care will be stored not only in a regional system, they will be synchronized with a Single state system in the field of health care. What too is very important if it is, for example, about rendering medical services, treatment, carrying out hi-tech medical actions not only in that region where there lives a person, but also in other places. Therefore it will not be necessary to carry with itself copies of the clinical record — documents will be already in base. Respectively, the medical organizations — at the consent of the patient, I want to pay to it attention because this information sensitive, it concerns health — will be able to exchange electronic medical records — the head of the government emphasized. |
He also noted that in information systems the list of medical services which are rendered to the patient that will allow to control better as well as what are spent for funds which are allocated for support of health care will be submitted. At the same time, electronic document management should help to cope better with queues and, eventually, "save our health care from excess pieces of paper", Dmitry Medvedev considers.
Meanwhile, on debugging of a system through the whole country time will be required, the prime minister is convinced.
The situation in our state different, digital inequality, a gap between regions, exists. But eventually we should come to a situation when such services are rendered in any point of our country electronically to any citizen of our state — the head of the government concluded. |

2016: The plan for selection of 5.62 billion rubles till 2025
The budget which the Government is going to select for achievement of the listed purposes is quite modest – 5.62 billion rubles till 2025. These investments are going to be made in development of a federal Uniform State Health Information System segment, reported TAdviser in the Russian Ministry of Health.
On development of information systems in regions federal financing is not provided. And it, according to the passport of the priority project, is one of the main risks.
Financial restrictions of territorial subjects of the Russian Federation when implementing actions for information technology implementation in health care can have an impact on project terms, said in the document
The previous stage of development of IT in health care was financed more generously - in 2011 - 2012 20.2 billion rubles of the budget of FCMIF and 9.2 billion rubles of budgets of subjects were provided. On these means basic informatization - purchase of computers and medical information systems was carried out to the medical organizations. The Ministry of Health directed about 4.9 more billion rubles to creation and implementation of federal applied components of Uniform State Health Information System.
One more risk of the priority project approved by Medvedev is connected with the fact that regional authorities will not be able timely to implement plans for development of Uniform State Health Information System (for minimization of this risk it is supposed to replicate the best practices in the lagging behind subjects).
Also risk is poor development of ICT infrastructure in regions, possible delays in adoption of law "About making changes in separate legal acts of the Russian Federation concerning application of information telecommunication technologies and introduction of electronic forms of documents in the field of health care" and the corresponding subordinate regulatory legal acts.
