Doctor Smart is a blockchain platform for remote consultations in the field of health. Doctor Smart has decision making support system which accompanies specialists at all stages of work. And the blockchain technology provides transparency of all financial calculations in a system.

Users can receive online consultations of therapists, nutritionists, endocrinologists, etc., to register in clinic or laboratory for analyses. By the beginning of 2018 the platform signed contracts for cooperation with 430 clinics and medical services of century Russia. In particular, were connected to the Doctor Smart platform "by Best Clinic", a medical neuronet "The third opinion", Labolatoriya Citylab (enters into Delrus Group of Yulaya Magadeev), etc. Using Citylab her patients will be able to make tests and to pay them through the application and also to receive the electronic version of results with comments of the therapist. The medical neuronet "The third opinion" is capable to recognize roentgenograms, MPT/KT and histologic researches using artificial intelligence.

Standard consultation lasts 20 minutes and costs the fixed 399 rub. There are premium rates which offer unlimited on time and the number of consultation. Service is presented in the form of mobile application and the website. Doctors of three specializations — the pediatrician, the therapist and the gynecologist are offered users. Doctors advise or in a text chat, or by means of a voice call, or by video conference. Also the additional service — interpretation of analyses is offered.

2020: Start of online consultations on a coronavirus

On March 26, 2020 the Smart company telehoney (Smart Medicina) reported that on Doctor Smart service free online consultations on a coronavirus are started. Since March 25, 2020 anyone can obtain up-to-date information about the current country situation, prevention of a coronavirus and rules of a quarantine. The qualified doctors will tell how to take care of themselves and the relatives in the conditions of a pandemic and what to do in case of symptoms of a virus. It is possible to get advice at any time — service works round the clock.

According to the management of Doctor Smart, similar initiatives from telemedicine services will help to reduce alarm level in society, and in certain cases will save citizens from need of visit of clinics and public places that is important in the conditions of observance of a quarantine.

2019: Skin scoring service

On August 13, 2019 the Doctor Smart service announced service start — scoring of skin based on AI. Users can scan the birthmarks (a pigmental nevus) for several seconds, having loaded a photo into the application. The neuronet will analyze the picture and will prepare the conclusion whether education is dangerous.

The Skinive AI project — the developer of the neuronets determining pigmental educations by the image became the partner of service. The principle of work is based on algorithms of artificial intelligence and computer vision. It is already the second product based on medical AI within service. Earlier Doctor Smart integrated a neuronet of Care Mentor AI for the analysis of radiological images. The neuronet gives the chance to the patient to analyze a research even before appointment or to receive the second opinion on already made diagnosis.

2018

The analysis of medical images of patients using a neuronet

Doctor Smart, blockchain service for online consultations in the field of health, and a medical neuronet "The third opinion" at the beginning of February, 2018 announced plans for joint start of service of the analysis of medical images for doctors and patients. It is supposed that this opportunity will appear in one of the first versions of Doctor Smart which will be started in the first quarter 2018.

So, any doctor or the patient who have scanned dabs of blood or marrow, the digitized pictures of an eye bottom or the roentgenogram of lungs, will be able to load them into the Doctor Smart application and using a medical neuronet to analyze "The third opinion" regarding presence of pathologies. The medical expert of Doctor Smart will be able to make consultation on the report of a neuronet and recommendations of further actions.

The analysis of medical images — one of the most relevant and important problems of medical diagnostics. According to representatives of The Third Opinion company, the medical neuronet is already capable to define the following pathological changes in blood cells and marrow:

• subpopulations of leukocytes;
• blastny cages;
• tumoral damages of marrow;
• diseases of platelets, etc.

Besides, the following pathological statuses of an organism are defined by a system in the analysis of images of an eye bottom:

• diabetic defeats;
• glaucoma;
• cardiovascular diseases, etc.

For the successful analysis a neuronet medical images should have the DICOM, TIFF or JPG format. The exact cost of service of the analysis of medical images using neural network in the company of "The third opinion" is promised to be reported according to the results of the closed beta testing.

The offered set of automatic researches is only the beginning of works on creation of a multi-layer intelligent system of the support of adoption of medical solutions based on application of methods of artificial intelligence when processing medical data. The number of the studied images will be constantly enlarged, and in combination with processing of other data the accuracy of researches will become more perfect — noted Anna Meshcheryakova, The Third Opinion project manager.

Cooperation with Citilab

At the beginning of February, 2018 it became known of cooperation of Doctor Smart  with network  of kliniko-diagnostic laboratories "Citylab". Thanks to telemedicine service patients of Citylab will be able to register in testing and to pay them and also to receive the electronic version of results with comments of the therapist.

As told the Kommersant newspaper in Doctor Smart, clients of Citilab will be able to register in the online mode in testing, to pay them, and then to receive results in electronic form and interpretation of the therapist. Services of service will be paid, their costs in the company did not open.

The telemedicine service Doctor Smart was connected to Citylab laboratories

In plans of developers to pass to completely electronic interaction in which it will not be required to patients the direction on diagnostics and the receipt in paper form will enough have in phone the Doctor Smart appendix with the paid service.

Our user will not need the paper directions, and he will know the cost of the necessary analyses at once. Everything that will need to be made after payment: select the nearest medical center "Citylab" or make tests in the convenient location, having called procedural crew — the development director of Doctor Smart Dmitry Popov reported.

According to the head of Citylab company Oleg Itskov, use of technology based on a blockchain of Doctor Smart will allow to receive to users other level of medical and laboratory service.

Doctor Smart will receive more benefit from cooperation with Citilab, than network of laboratories, the director of a business activity and marketing of Medsi group Alla Kanunnikova considers. She explains it with the fact that the Doctor Smart project has no own customer base yet. The service in itself is not something new, Alla Kanunnikova claims.

According to her, the law on telemedicine still the progressive tense will be improved and the leader in a segment will be able to become that company which will manage to adapt to it already in the course of work.^[1]

2017: Creation

In December, 2017 it became known of start of service of online consultations with doctors of Doctor Smart. The Smart Telehoney company  which in equal shares was founded by the son of the co-owner of Meditsina clinic Pavel Roytberg and the operating officer of Mail.Ru Group Vladimir Nikolsky manages the project.

Users will be able to receive online consultations of specialists (therapists, nutritionists, veterinarians, etc.), and then — to register in clinic or laboratory for analyses. Doctor Smart will cooperate with the clinics, the fitness centers and doctors having the diploma, the operating certification and Doctor Smart which passed test Roytberg told Vedomosti.

Remote consultations with doctors in Russia were legalized by the law on telemedicine

For the analysis of medical records of clients and financial transactions in Doctor Smart artificial intelligence technologies and a blockchain are used.

According to Vladimir Nikolsky, he announced Mail.Ru Group investments into Doctor Smart. The representative of holding noted that the company "not against" when employees invest in perspective projects. Mail.Ru Group develops  Health of Mail.Ru service within which, in particular, it is possible to order drugs and to consult with specialists online. 

The cost of a basic service of Doctor Smart will begin from 300-400 rubles. For comparison: at The Doctor Is Near service consultation of the therapist costs 500 rubles, and at "Yandex" repeated "visit" to the specialist will cost 499 rubles whereas primary — 99 rubles. Start of Doctor Smart is planned for 2018, but more exact terms of the beginning of work of Internet service are not called.

Investments into the project are estimated at several million dollars. Roytberg and Nikolsky made own investments in him.

Remote consultations with doctors became possible after legalization of the law on telemedicine: doctors had an opportunity to write out recipes and to communicate with patients remotely.^[2]
