The TeleRehab application to support medical decision-making to restore patients after a stroke was developed by employees of the University of MISIS and the Research Institute of Health and Medical Management of the Moscow Department of Health (NIIOSMM DZM). The program based on algorithms and artificial intelligence gives the most suitable recommendations for telerehabilitation for each case. MISIS announced this on October 22, 2024.

Most physicians involved in rehabilitation of patients after stroke are well acquainted with traditional treatments. However, due to the high workload and rapidly changing technologies, specialists may not know the latest trends in the health care system. Developed software (software) makes it easier for doctors to work, reducing the risk of errors.

We examined technical and digital solutions that are used in medical rehabilitation after stroke and developed software to help physicians implement best practices for telemedicine rehabilitation of stroke patients. The system issues recommendations on remote therapy, technical equipment and software necessary for its implementation, - explained the co-author of the development, candidate of technical sciences, associate professor of MISIS University Alexander Nikolaev.

To accurately prescribe therapy, you need to assess the well-being and rehabilitation potential of patients. To do this, TeleRehab introduces the results of post-stroke-specific medical tests and scales, for example, Fugl-Meyer, Romberg, the Montreal Cognitive Assessment Scale, etc. The system processes information and issues recommendations for the preparation of a therapy plan - exercises, their frequency and intensity, as well as the duration of treatment. The application will tell you what additional medical and digital devices are needed for therapy, including for rehabilitation at home.

To model clinical cases, the developers used a randomization method using synthetic data. For software testing, scenarios were randomly generated taking into account age, sex, dysfunction, clinical stroke stage, as well as medical test results for 180 virtual patients. The system offered recommendations for drawing up effective treatment plans for all cases. The study results are described in detail in the scientific journal Life (Q1).

Digital health care is a trend in the development of medicine. Our software allows doctors to make operational decisions for the organization of restorative treatment of post-stroke patients using telemedicine technologies based on a personalized approach and the principle of continuity of rehabilitation. The recommendations are based on the results of current clinical studies and practices. This contributes to improving the quality of medical care and increases the number of those who need it, - said the co-author of the development, graduate student of NIIOSMM DZM, leading analyst at the N.I. Pirogov Russian Ministry of Health Vitaly Nikolaev.

The program also allows medical institutions to analyze data from electronic medical records and give recommendations to doctors, sending tips online. Automation of the procedure reduces the associated burden on health care professionals, allocates more time to other patients and increases the personalization and effectiveness of treatment.

According to the creators, in the future, the TeleRehab program can be finalized for specific customer requests and easily integrated into the information systems of healthcare organizations.
