Implementation of an IT system for accounting for newborns in Krasnoyarsk and neonatal screening control

On August 16, 2023, TechLAB announced that a centralized regional information system has been introduced in the Krasnoyarsk Territory, which allows monitoring compliance with the deadlines for each stage of neonatal screening. By incorporating this digital solution into mass survey practice it became possible for newborns to monitor the time of diagnosis of serious diseases and increase the efficiency of providing data on the health of each baby in the region. The system was developed by TechLAB (St. Petersburg).

According to the company, neonatal screening is a mass examination of newborns, which is carried out to early identify possible hereditary and congenital diseases. To do this, babies take blood from the heel: it is examined in the laboratory for 36 diseases, such as cystic fibrosis, spinal muscular atrophy, congenital hypothyroidism. These diseases can pose a serious threat to the life and health of the newborn, however, with early detection, the doctor can prescribe additional studies in time and select treatment to minimize the impact of the disease on the child's life.

Analysis

True, for this it is important to comply with the deadlines for each stage of neonatal screening prescribed by the Ministry of Health of the Russian Federation: for example, blood samples from the heel should be taken from full-term children within 24-48 hours after birth, blood samples should be transported to the laboratory in no more than two days, and the screening study itself should take no more than three days.

Since, from 2023, the number of diseases that are included in the screening program, increased from 5 to 36, and the burden on medical organizations, laboratories and other institutions dealing with the documents of newborns have grown, then the need for such an IT solution that aggregates all the necessary medical information and allows you to control the timing of stages and key indicators as part of neonatal screening at the region level became obvious. In addition, the implementation of automated monitoring tools will eliminate errors that could occur in the event of manual data entry.

A similar decision for August 2023 was implemented in the Krasnoyarsk Territory: it allows you to track the timing of all stages of neonatal screening, which means that you can monitor the efficiency of diagnosis of the diseases included in it. The development of the solution for seven months was carried out by the St. Petersburg IT company "TechLAB," specializing in the creation of digital health services for private and state medical organizations.

We have developed an electronic register of newborns with the possibility of generating statistical reports, with the help of which the timing of neonatal screening is monitored in the Krasnoyarsk Territory. Their compliance is really very important: a lot depends on when the newborn's blood sample is taken, when it is delivered to the laboratory, when the study is carried out and the results are obtained - the child's health and quality of life in case any of the serious diseases are revealed as part of the screening.

The neonatal registry described is based on data from several medical documents. So, first, an obstetrician-gynecologist forms a medical birth certificate, initiating the possibility of state registration of the birth of a child, then a neonatologist in his medical information system (MIS) creates a referral to undergo screening for a particular baby, after which this document, together with test strips with blood samples, falls into laboratory, from where the conclusion on neonatal screening comes in the form of another electronic document.

These and other information are received in the register of newborns in various formats: XML CDA R2 (SAMD), JSON FHIR, etc., from the information systems of institutions of the Krasnoyarsk Territory: the Regional Archive of Medical Documents, MIS operating in the medical organizations of the region, the Regional Laboratory Information System and the Demography Information System. Data from the registry, in turn, is transmitted in the form of a SAMD (for which, if necessary, data are converted) to a vertically integrated medical information system (VIMIS) for obstetrics, gynecology and neonatology (AKiNEO).

In the Krasnoyarsk Territory, the register of newborns for monitoring the timing of neonatal screening is already actively used: it already stores data on more than 13,720 babies obtained from 70 institutions in the region. Its implementation allows not only to keep records of newborns and control the timing of the screening stages, but also to control the quality of the input data.

For example, you can make a mistake in the number of the child's birth certificate or release two identical non-screening directions - in this case, the data will be doubled. Our system uses control algorithms to timely detect such abnormal situations and promptly inform responsible employees. So errors can be quickly noticed and make the required corrections to the documents, which, in turn, ensures that the screening for the child will be carried out on time and carefully.

The neonatal registry itself and statistical reports on the timing of neonatal screening were created on the basis of the Patient's Multi-Passport, the flagship development of TechLAB, a single platform for creating centralized subsystems and registers for any nosologies.
