17 million diagnoses
The service for making a final diagnosis, developed jointly by the company, SberMedII the Laboratory artificial intelligenceSberbank and, Government of Moscow has been introduced into the work of all adults policlinics in Moscow since the end of 2023. Sberbank announced this on October 17, 2024.
During this time, the diagnostic assistant helped therapists and GPs make more than 3 million final diagnoses according to the electronic medical record (EMR). In addition, the digital assistant to the TOP-3 doctor has been working in the capital's polyclinics for 4 years, with the help of which more than 14.4 million preliminary diagnoses have already been made at primary appointments.
The diagnostic assistant works as follows: the doctor determines the final diagnosis, AI analyzes the patient's data in parallel and also forms the diagnosis. If it coincides with the opinion of the doctor, the service informs that it supports the specialist in his decision. If the diagnosis is different from the one given by the doctor, the system signals it and offers its own option.
{{quote 'author=said Sergei Zhdanov, Director of the Health Industry Center of Sberbank. | Artificial intelligence is a technology that has already shown effectiveness in solving medical problems. The Sberbank health industry and partner companies develop and implement services in which AI is involved in decoding images, preparing radiological conclusions and helping primary health care doctors. Together with the Moscow Government, we have introduced two diagnostic AI assistants in all metropolitan polyclinics to make preliminary and final diagnoses to patients. During the application of these decisions, more than 17 million diagnoses have already been made. This is a huge figure, behind which there are significant savings in doctors' time to perform routine actions and the ability to focus on complex medical cases. We work closely with expert physicians and healthcare representatives to create and improve products and services for physicians and patients,}}
{{quote 'author=noted Anastasia Rakova, Deputy Mayor of Moscow for Social Development. | The service for making final diagnoses was trained on a large amount of data - on records of 30 million patient visits in two years. When developing it, the expert opinion of the capital's doctors was also taken into account. For a year now, the service has been used in all adult Moscow polyclinics, and we continue to regularly monitor its reliability and accuracy, including with the involvement of experts - our doctors. This is the standard practice of monitoring all artificial intelligence products operating in the city. So, at the end of the year, work on a number of diagnoses required further training of the model. Now doctors have an updated service with an accuracy of diagnosis above 80%, for some diagnoses it already exceeds 90%. The service helps at the reception to establish the most common and significant diseases among Muscovites, most often requiring dispensary observation, for example, diabetes mellitus, arterial hypertension, asthma, ulcer, rheumatoid arthritis and others. When the patient re-visits the doctor, the diagnostic assistant analyzes all data from the electronic medical record for the last 2 years, including the date of admission, including the results of laboratory and instrumental studies, discharge epicrises, protocols of examinations of specialists,}}
Staging with the use of an assistant doctor "Top-3" 14 million diagnoses
The digital assistant developed by Sberbank and the Moscow Government helped the therapists of the capital's polyclinics to make 14 million diagnoses. Sberbank announced this on May 22, 2024.
A line of digital medical assistants in diagnostics has been introduced in all adult polyclinics in the capital, developed jointly by the Government, Moscow the company SberMedII and the Sberbank Artificial Intelligence Laboratory. The digital assistant to the doctor "TOP-3" has been working in Moscow polyclinics since October 2020, during which time 14 million preliminary diagnoses were made with his help.

{{quote 'author=noted Anastasia Rakova, Deputy Mayor of Moscow for Social Development. | All adult urban polyclinics use a medical decision support system that works on the basis of artificial intelligence algorithms. It was developed several years ago in conjunction with SberMedII and the Sber AI Laboratory. One of our services, the digital assistant "TOP-3," analyzes complaints about the patient's health and, based on the information received, offers the three most likely preliminary diagnoses. The doctor can agree with one of them or put his own. The service identifies 95% of the most common diseases. In May 2024, he helped Moscow doctors make 14 million preliminary diagnoses,}}
2023: Development and implementation of the Aida diagnostic assistant
In September 2023, the Moscow mayor's office talked about the creation of an AI tool to help doctors in the capital's polyclinics. The diagnostic assistant "Aida," developed jointly with "Sber," helps therapists and general practitioners in making the main final diagnosis based on the data of the electronic medical record (EMR) over the past two years.
During the patient's repeated visit to the doctor, the service analyzes all text data from the last two years, including the date of admission collected in the electronic medical record (EMR), including complaints and history, medical history, results of the initial examination, laboratory and instrumental studies. Based on these data, the model helps doctors in making the main final diagnosis out of the 95 most common and significant diagnoses, often requiring dispensary observation.
The Aida diagnostic assistant works as follows: when the doctor determines the final diagnosis, the AI analyzes the patient's EMR in parallel and also forms the diagnosis. If it coincides with the opinion of the doctor, the service informs that it supports the specialist in his decision. If the diagnosis is different from the one given by the doctor, the system signals it and offers its own option. The task of the service is to additionally "insure" the doctor by providing him with an independent "second" opinion. In this case, the final decision is always made by the doctor.
The press service of the mayor's office explained that the new system module is connected in the event that, during filling out the examination protocol in the unified medical information and analytical system (EMIAS), the therapist decides to make a final diagnosis to the patient.
The model was developed with the participation of experts from SberMedII (part of the Sberbank health industry) and the Sber Artificial Intelligence Laboratory. The diagnostic assistant was trained by Moscow doctors of the highest level on a large amount of data - on records of 30 million patient visits in two years. During the experiment with the involvement of 38 doctors of the highest category and the status of "Moscow Doctor," the service has already demonstrated an accuracy of more than 87% in determining the 95 most common diagnoses. As of September 2023, the service already in 40% of cases shows the doctor a hint at the appointment, thereby helping to make a final diagnosis. In the future, it is planned to expand and further learn the model to analyze more diagnoses. The system is piloted in 24 metropolitan polyclinics. By the end of September, it will be introduced into all adult clinics in Moscow.
We see huge potential in the use of artificial intelligence technologies for the development of Moscow health care. Already today, AI has been introduced into the routine practice of doctors of urban medical institutions. We move on and launched a new module of the medical decision-making support system in 24 polyclinics, - said Moscow Deputy Mayor Anastasia Rakova, whose words are quoted by the press service of the Moscow government on September 7, 2023. |
{{quote 'author=noted Sergei Zhdanov, Director of the Health Industry Center of Sberbank. | "AIDA" is a system for deep diagnosis and monitoring of the patient's condition. The creation of this model on the basis of AI is already the second joint project of the Government of Moscow and Sberbank in the field of digitalization of the polyclinic link. Thanks to cooperation since 2020, at the initial appointment in all polyclinics in Moscow, the smart assistant to the doctor "TOP-3" helps specialists in making a preliminary diagnosis. And now, after the examinations, based on all the data from the EMR of the patient, "AIDA" will help in making the final diagnosis,}}
The joint development of Sberbank and the Moscow Government also contains a number of AI-based solutions that are designed to increase the speed of diagnoses. This is a service predictive analytics for making a newly diagnosed out-of-dose diagnosis based on electronic medical record data, as well as a model predicting the risk of developing/in a strokeheart attack patient in the short term (from 6 to 18 months) according to EMR data.
