2022: Completion of the implementation of the Oncology subsystem in the entire Murmansk region

On June 9, 2022, the TechLAB company announced that the Oncology subsystem was introduced and operates in medical organizations of the Murmansk region: it combines data on patients with cancer coming from all medical organizations in the region. The introduction of this subsystem made it possible to optimize the quality of medical care for patients with oncology and reduce the burden on doctors: for example, a more understandable procedure for routing oncology patients was built in the region, recommendations for the treatment of each such patient were personalized, and a constant and prompt exchange of specialized medical information of regional medical institutions with the federal system VIMIS "Oncology" was ensured. The project was implemented by the IT company "TechLAB" together with the companies "Netrika Medicine" and "Solution."

According to the company, since 2019, the Murmansk region has been implementing a regional project "Creating a single digital circuit in healthcare on the basis of a single state health information system (Uniform State Health Information System)." He suggests that an information system should be introduced in medical organizations in the region that meets the requirements of the Russian Ministry of Health - the so-called centralized subsystem. It is also designed to transfer medical data to the federal information system - VIMIS (vertically integrated medical information system) and promptly receive clinical recommendations from it. The main goal of these digital transformations is to optimize the efficiency of the healthcare system.

The creation of centralized subsystems is primarily focused on providing more accurate and prompt diagnostics and treatment for the most common nosologies in Russia with a high mortality rate, for example, cardiovascular diseases. Oncological diseases also belong to the same list.

The initiator of the project to create such a subsystem for oncology in the Murmansk region was the Murmansk Regional Oncological Dispensary (MOD) as the main specialized institution in the region. The developer of the subsystem was the St. Petersburg IT company "TechLAB."

We have developed a centralized subsystem for oncological medical care (otherwise - a centralized subsystem "Oncology"). It is designed to help doctors in making decisions on patients with oncology: since this group of diseases is characterized by a high mortality rate, the level of which directly depends on the timing of the detection of the disease, the diagnosis and treatment of cancer patients require compliance with clinical recommendations and close attention from the doctor. The creation of the regional subsystem "Oncology" and its integration with the federal system "VIMIS" are focused on achieving these goals.

The development of the centralized subsystem "Oncology" was carried out by the team of the company "TechLAB" in stages for 2 years, the project was launched in the third quarter of 2020. As of June 2022, the system has been implemented throughout the Murmansk region and is available from any medical organization in the region.

The Netrika Medicine team was responsible for transferring structured information from the Murmansk Regional Oncological Dispensary to the subsystem, and the Decision company was responsible for transferring the necessary information about the patient from the medical information system to the regional services of the Unified State Health Information System (Uniform State Health Information System).

The subsystem developed by TechLAB specialists optimizes the work with electronic documentation for medical specialists, acting on the principle of single input and repeated use of data. Now structured medical information (SMS), necessary for further transmission to VIMIS "Oncology," is formed directly in the subsystem. This allows you to reduce the burden on doctors and minimize it on the side of medical organizations in the Murmansk region.

Thanks to the integration of the Centralized Subsystem "Oncology" with the regional electronic medical record of the patient and the federal VIMIS system, we can now fulfill the requirements for collecting and providing a large amount of information to VIMIS. We are faced with a difficult task: it is necessary not only to collect and transmit to all interested parties information on the course of diagnosis and treatment of cancer patients, but also to make sure that the attending physician does not need to allocate additional time for this. As you know, finding it during working hours is not easy, and we categorically did not want this to affect the interaction with the patient and, as a result, the quality of treatment.

Thus, the introduction in medical organizations of the Murmansk region of the centralized subsystem "Oncology," developed by the specialists of "TechLAB," allows you to make decisions on the treatment of each patient, save the doctor's time at the appointment, and promptly receive up-to-date clinical recommendations from the relevant ministry.

2021: Implementation of VIMIS "Oncology"

As part of the implementation of the federal projects "Creating a single digital circuit in healthcare on the basis of Uniform State Health Information System" and "Combating malignant neoplasms" GOBUZ "Murmansk Regional Oncological Dispensary" decided to introduce a centralized regional subsystem "Oncology." This was announced on July 9, 2021 by Netrika Medicine.

The regional subsystem "Oncology" also interacts with the MIS "Ariadne" in order to transfer laboratory tests from the medical organization to the relevant services.

Information has already been sent to the system about 13,000 patients with PRN or pre-neoplastic diagnoses.

TECHLAB specialists were responsible for the development of the subsystem. Its main tasks are to manage the routing of patients with malignant neoplasms, monitor the timing and quality of the medical care provided to them, as well as ensure the continuity of treatment. Also, the IT solution provides oncologists with recommendations for prescribing therapy, taking into account the individual clinical picture of each patient.

One of the key tasks of the project is the implementation of services for interaction with VIMIS "Oncology." It is extremely important to minimize work on the side of medical organizations and implement the centralized formation of SMS (structured medical information) directly in the Oncology subsystem. This architecture allows you to significantly reduce the burden on doctors in the process of collecting the data necessary for VIMIS "Oncology." Doctors continue to work with the same documents in the Ariadne medical information system, the workflow does not change, the list of parameters is only slightly expanding. At the same time, the document flow thanks to the Ariadne MIS is transferred to an electronic exchange format, which also increases efficiency and reduces the burden on medical personnel. Centralized management of directories is provided using the regional NSI subsystem and mapping of directories in the Oncology subsystem. As of July 2021, the regional subsystem "Oncology" is registered in the VIMIS test circuit and analyzes all trigger points and transmits the first five SMSs. To ensure the completeness and quality of data received by the regional segment of the Uniform State Health Information System of the Murmansk region, the regulation on the transfer of information about patients with PHO to the Uniform State Health Information System MS was adopted, monitoring tools were created to assess the compliance of the regulations with medical records received from medical information systems.

The Oncology subsystem includes another component - the Oncology Early Detection Solution. This is one of the latest developments in TechLAB, which helps primary care doctors assess the likelihood of a patient having a malignant neoplasm (as of July 2021, the system predicts 17 main types of cancer) and decide on its direction for further diagnosis. According to the project participants, in 2021, the Onconastiveness component is planned to be scaled up at the primary medical organization.

The Netrika Medicine team implemented an information exchange protocol between the subsystem and the services of the regional segment of the Uniform State Health Information System of the Murmansk region, which made it possible to ensure the transfer of structured information from the oncology dispensary to the Oncology subsystem: based on them, the IT solution forms a patient register. The regional service of the Doctor's Portal was also integrated with the Oncology subsystem.

The specialists of the company "Solution" have expanded the interface of the medical system "Ariadne" to ensure the accounting of the necessary information about the patient and their transfer to the regional services of the Uniform State Health Information System. The access of a doctor from the Ariadne MIS to aggregated information about the patient's malignant formation in the Oncology subsystem was implemented by opening a web application using a link from the MIS.

To achieve the goals of introducing the regional subsystem "Oncology," among other things, we were faced with the task of automating the cytological laboratory of the dispensary, which is an important link in the study and detection of neoplasms of residents of the entire Murmansk region. As a result of the work, it became possible to automatically transfer information from the laboratory to regional REGIZ services and further to specialized services, such as the regional subsystem "Oncology." The joint work of the employees of LLC "Solution" and the dispensary made it possible to translate the main processes into digital format, as well as to carry out full and correct data transfer to the necessary services. The results of the work allow medical personnel and patients to be confident in the results obtained, - explains Irina Khusainova, General Director of Decision LLC.

One of the principles of our work is open cooperation with all developers of information systems for medicine. The joint project in the Murmansk region was implemented in just three months - a time frame extremely tight for this level of complexity - due to the use of the FHIR standard and the capabilities of the N3 integration platform. Healthcare. Adding a regional subsystem to a single region digital loop does not create additional data entry interfaces for medical organizations. The doctor enters information into the MIS and information through the N3 platform.
