The scheme for obtaining an electronic prescription in Moscow is quite simple: the doctor writes out a document in electronic form, entering the data in EMIAS on his computer, then the system transfers the appointment to the pharmacy at the clinic. The patient-beneficiary will only have to get the drug there, having with him a printed prescription and a compulsory medical insurance policy, which the pharmacist enters into the system by scanning the barcode. Thus, a request is formed to the EMIAS for confirmation of the prescribed prescription, an employee of the pharmacy sees the appointment of a doctor, and issues a medicine.

Mishustin allowed doctors of private clinics to write prescriptions online

In July 2023, Prime Minister Mikhail Mishustin signed a decree that established a new experimental legal regime to expand the possibilities of remote interaction between a doctor and a patient using telemedicine technologies. As part of the experiment, doctors at private clinics write prescriptions online.

According to the document, 15 medical organizations with a license and IT capabilities will take part in the experiment. Among them are Sberbank, Russian Railways-Medicine, Medsi Group of Companies, Doctor Nearby, M-Line, RenKlinik, European Medical Center. Minister of Economic Development Maxim Reshetnikov noted that in the future this list may be replenished.

The doctor will be able to form a prescription in the form of an electronic document only for the diagnosis established earlier at a face-to-face appointment. In order for the specialist to have access to the necessary data, a clause on consent to the exchange of data in the Uniform State Health Information System will be added to the contract between the patient and the clinic.[1]

According to him, by February 2023, you can get drugs on electronic prescriptions in more than 2.5 thousand pharmacies. Electronic prescriptions with a QR code are accepted in all pharmacies subordinate to the Moscow Department of Health, as well as in a number of commercial pharmacy chains connected to a single medical information and analytical system (EMIAS). If the smartphone is discharged or broken, the patient can print the QR code of a preferential prescription in the information kiosk of the clinic by scanning the compulsory medical insurance policy. Patients who do not use prescriptions electronically can get a traditional paper form from a doctor. It will also be needed if the townspeople want to purchase medicines from a pharmacy that is not connected to EMIAS.

Moscow will begin to issue preferential electronic prescriptions for up to a year

In Moscow, they will begin to issue preferential electronic prescriptions for up to a year. This became known on November 24, 2022.

Patients who require long-term course treatment and who are entitled to receive drugs for free or at a discount will be able to receive a preferential prescription with a validity period of up to a year.

It will be possible to receive medicines for them once every one or three months without the need to renew the prescription in a medical organization. The frequency of dispensing drugs and the duration of course treatment is determined exclusively by the attending physician. Such an approach will free Muscovites from unnecessary visits to the clinic to obtain prescriptions, said Anastasia Rakova, Deputy Mayor of Moscow for Social Development.

We have launched this pilot project for the convenience of Muscovites in need of long-term drug treatment who have the right to receive preferential drug provision. Preferential electronic prescriptions with a validity period of up to a year are prescribed to patients who are entitled to receive drugs at the expense of the city and federal budgets. Long-term course treatment can be prescribed to patients with certain chronic diseases, for example, diseases of the circulatory systems, musculoskeletal and respiratory systems, endocrine diseases, as well as those who have a high risk of their development or are in a recovery period after suffering acute diseases. You can get preferential medicines with a validity period of up to a year using the QR code of an electronic prescription or using a compulsory health insurance policy (compulsory medical insurance).

The issued prescription with a prolonged validity period is available to patients in the electronic medical record in the "My prescriptions" section on mos.ru mobile application and in, as EMIAS.INFO well as in the infomat of the clinic - it will be marked with a special symbol. When viewing the prescription, information such as the possible frequency of receipt of drugs (once a month or every three months), the amount of the drug to be dispensed, the schedule of dispensing the drug, the status of the current and/or expired period are displayed.

A history of servicing a preferential electronic prescription issued for a long period (the date of its service, the name and address of the pharmacy, the trade name of the drug issued and the number of packages received) will also be available.

Electronic prescriptions with a QR code are written in all city polyclinics from February 2021. These include both discounted e-prescriptions and full-cost prescriptions. Muscovites can receive medicines prescribed by a doctor by presenting a QR code on the screen of a mobile device. This option is available in all pharmacies of the city's Department of Health and in hundreds of commercial locations. In addition, in case the smartphone fails, is lost or discharged, the QR code of the preferential prescription can be printed in the information kiosk of the clinic by scanning the compulsory medical insurance policy. Also in pharmaceutical points at city policlinics instead of the electronic recipe with the QR code the patients can show the policy of compulsory health insurance for receiving preferential medicine [3].

Creation of a federal register of electronic prescriptions

On April 14, 2022, it became known about the creation in Russia of a federal register of medical appointments, which will collect all information about the issued electronic prescriptions for drugs. At the end of April 2019, Croatia became the fifth country to join the system of cross-border exchange of electronic prescriptions with other EU member states. Now Croatian patients will be able to receive medicines using electronic prescriptions prescribed in their home country, even traveling to Finland, Estonia, Luxembourg and the Czech Republic.

An electronic system health care that allows European countries to exchange digital recipes was launched in early 2019. Its first participants were Finland and Estonia. It is known that in the period from January to April 2019, more than 1000 Finnish patients traveling in Estonia received medicines in local pharmacies with prescriptions issued in Finland. Such records, the European Health Record, should be kept on all EU resident patients and could form the basis for the next step of eHealth reform - the creation of an electronic medical records exchange system.[4]

Ministry of Health allowed doctors to write electronic prescriptions

From April 7, 2019, all Russian doctors will begin to write out electronic prescriptions to patients in accordance with Order of the Ministry of Health No. 4n of January 14, 2019. Document entitled "Order on Approval of the Procedure for Prescribing Drugs, forms of prescription forms for medicinal products, the procedure for issuing these forms, their registration and storage "is posted on the official portal of legal acts. The transition to the issuance of electronic prescriptions is due to federal law No. 242-FZ "On the use of information technologies in the field of health protection," which entered into force in January 2018. The text of the law approved the possibility of issuing prescriptions in electronic form if the patient or his legal representative gives his consent to this.

2018: Muscovites were given more than 45 million electronic prescriptions in four years

At the beginning of 2018, the capital's doctors issued more than 45 million electronic prescriptions to patients, of which 43 million were preferential, citizens can receive medicines for them free in pharmacies at clinics. This opportunity appeared among doctors in 2014, after the introduction of this service into the Unified Medical Information and Analytical System (EMIAS). The project was developed and implemented by the Department of Information Technologies of the city of Moscow together with the Department of Health of the city of Moscow.

On average, Moscow doctors write more than 300 thousand electronic prescriptions per week. The EMIAS service allows a doctor to issue an electronic prescription seven times faster than if he did it by hand. And the specialist can devote the saved time to his patients.

You can check information on an electronic prescription using infomats in any polyclinic in the city or using the "Appointment to the Doctor" service on the portal mos.ru. 2014: More than half of Moscow's state-owned clinics have mastered the design of recipes in electronic form

By the end of 2014, more than half of the state polyclinics in the capital had mastered the design of prescriptions in electronic form. This component of the Unified Medical Information and Analytical System (EMIAS) is already used by doctors in 370 clinics. It is planned to complete the implementation of an electronic prescription in all children's and adult polyclinics in Moscow in the first quarter of 2015.

The demand for the service is confirmed by statistics: by December 2014, more than 3.7 million electronic prescriptions had already been issued, of which more than 85% were served. The new format for issuing prescriptions is very convenient for preferential categories of citizens who regularly receive medicines for free or with a 50% discount.

The advent of an electronic prescription will simplify the lives of patients, facilitate the work of doctors and bring significant benefits to the city. Previously, it took up to 6 minutes for a doctor to manually write one prescription on a typographic form of strict reporting. Now this process takes no more than a minute, since some of the patient data is already contained in the system and they do not need to be filled in again every time. The introduction of new technologies can save 600 thousand person-hours of doctors' time per year. This time can be spent by the doctor on a more attentive examination of the patient and diagnosis.

In addition, with the transition to paperless prescription design, it will be possible to significantly reduce printing costs. Today, according to the requirements of the law, even having written out the appointment in electronic form, the doctor must print it out. In the case of preferential prescriptions, you can do without this, because the patient receives medicines at the pharmacy at the clinic connected to the EMIAS, and the pharmacist sees the doctor's appointments in the system.

"Now the prescription is defined by federal law as" written prescription of a drug, "and this outdated wording does not allow you to refuse to print unnecessary papers and translate the preferential prescription completely into electronic form, says Vladimir Makarov, Deputy Head of the Moscow City Information Technology Department, - "It is necessary to amend the existing regulatory framework as soon as possible, and now active work is underway on this at the level of the Ministry of Health of the Russian Federation, the Government of Moscow and the Department of Health of the city of Moscow."

12.5 million preferential prescriptions are written and printed in Moscow per year, which is a stack of paper more than 1.5 km high - this is higher than 3 Ostankino TV towers.
