In early October 2020, the Ministry of Justice of the Russian Federation registered an order of the Ministry of Health regulating the provision of dental care to adults.

Medical care for the adult population is provided for dental diseases of the teeth, periodontal, mucous membrane of the mouth, tongue, salivary glands, jaws, including:

  • carious, non-carious, and other dental lesions;
  • acute, chronic and specific inflammatory diseases, acute and chronic trauma, acquired defects and deformities, periodontal cancer, oral mucosa, tongue, salivary glands, jaws;
  • anomalies and defects in the development of teeth, dentition, jaws, their prerequisites and consequences.

The procedure for providing dental care to the adult population of the Russian Federation has been approved

According to the document, in the presence of medical indications for the provision of medical care for dental diseases, the paramedic, after carrying out measures aimed at eliminating pain, sends the patient to a medical organization to provide primary specialized medical care to a dentist, dentist, dentist, surgeon, dentist, orthopedic doctor, orthodontist (hereinafter referred to as dental doctors).

The prevention of dental diseases is carried out by medical organizations providing medical care to the adult population for dental diseases, and includes preventive measures, including preventive medical examinations, which are recommended to be carried out 1 once every 6 months.

In addition, as reported in the order of the Ministry of Health of the Russian Federation, medical assistance to the adult population for dental diseases is provided taking into account the standards of medical care and on the basis of clinical recommendations.[1]

Statistics of oral diseases

According to the World Health Organization at the beginning of 2019, worldwide 60-90% of school-age children suffer from tooth decay, and almost 100% of adults already have dental decay. Oral health statistics also speak of other disappointing facts: severe periodontitis, which leads to tooth loss, is present in 15-20% of people aged 35-44 years. In addition, approximately 30% of people aged 65-74 do not have natural teeth, and factors such as unhealthy eating, alcohol consumption, smoking and poor oral hygiene are risk factors for developing oral cavity diseases.

In the coming years, the market will develop at the expense of two industries: orthodontics and aesthetic orthopedics. Until the 2000s, the culture of oral care in most people was limited to timely brushing of teeth. This was due to the fact that most of the treatments were long and painful, and there were no other ways and quality anesthesia. Moreover, in the 90s, many people simply could not afford treatment from a dentist and went to an appointment when the pain was already unbearable, and the clinical case was more neglected[2]

Now the situation has changed radically. New treatments allow doctors to perform procedures painlessly, atraumatically and quickly. In addition, active education of the population made it possible to convey to people information about how important it is to regularly visit a doctor and comply with basic recommendations for the prevention of dental diseases.

The number of acute pain decreased, and aesthetic dentistry became a more frequent request from patients. Social media and fashion brands promote naturalness, patients become more demanding on the appearance of orthopedic designs, want veneers and crowns to look like natural teeth. Orthodontics can also satisfy the aesthetic needs of patients: according to individual indications, it became possible to use eliners - removable "invisible" polycarbonate caps that can align the dentition and bite without "damage" to the patient's appearance.

In mid-September 2022, it became known that researchers from Russia and Egypt developed an innovative coating that managed to surpass natural enamel in characteristics. Hydroxyapatite is used as a starting point for the technology. This compound is a major component in bone tissue and mineralized human and animal tissues.

The researchers doped hydroxyapatite with a complex of amino acids that naturally contribute to the restoration of bone and muscle structures such as lysine and arginine, resulting in a mineralized layer that resembles the main component of natural enamel in properties. The material was then applied to healthy teeth so the team could observe its ability to bind to real dental tissue.

Created a special dental coating that exceeds the hardness of natural tooth enamel

To study the properties of the new material, scientists used chemical imaging, field emission electron and atomic force microscopy. The thickness of the mineralized layer was found to be between 300 and 500 nm, and the nanocrystalline structure corresponds to the structure of apatite crystals in natural enamel. Importantly, the new coating demonstrated impressive strength and even greater hardness than natural enamel when measured at the nanoscale.

Enamel, being a very thin but very strong tooth coating, plays an important role in protecting against tooth decay and destruction, but it is also one of the only tissues in the body that cannot recover. This means that its lifelong erosion can lead to serious dental problems, but we have already seen some inventive approaches to repairing enamel, such as using low-power lasers to stimulate stem cells or toothpaste with peptides.

Offering another potential solution, the team imagines that its technology will help restore enamel erased by erosion or abrasion. But scientists also hope to refine it by exploring the potential for its application to address larger defects such as cracks and fractures.[3]
