1 million health workers in Russia for the year received social payments for 74 billion rubles, except for salaries
In 2023, more than 990 thousand medical workers in Russia received special social payments totaling ₽74 billion. This became possible thanks to a special program implemented by the Ministry of Health of Russia. The data on payments were announced in the spring of 2024 at a meeting of the board of the Russian Ministry of Health.
According to Interfax, medical workers received payments under a program aimed at supporting specialists working in primary health care. In particular, amounts from ₽4,5 thousand to ₽18,5 thousand were paid, depending on the specialty and region of work.
As follows from the report of the Ministry of Health, special social support was implemented in order to retain and attract health workers to state medical institutions. In addition to federal programs, regional measures are also being introduced to increase the attractiveness of the profession.
The Federal Compulsory Health Insurance Fund (MHIF) clarified that payments were sent to 989 thousand medical workers, which is less than 1 million with the planned 1.2 million. In total, ₽75,9 billion were spent on payments, although initially it was planned to allocate ₽152,4 billion. These data were obtained from the foundation's 2023 budget performance report.
Of this amount, 65% were received by doctors working in the compulsory medical insurance system, 70% - by nurses and 47% - by junior medical personnel. It is noteworthy that only ₽24,8 million were allocated for payments to doctors for onconference, which is 45.9% of the approved volume of ₽54 million. The main reasons for the inadequacy of funds were cases of untimely establishment of dispensary observation and an insignificant amount of payments for specific health workers (₽250 and ₽500).[1]
Reduction in the number of doctors to 41.5 per 10 thousand population

For example, the provision of the population with doctors providing primary health care amounted to 23.27 people. per 10 thousand population with a planned annual value of 22.20 (104.82% of achievement), the indicator of the number of doctors working in state medical organizations is 608.363 thousand people. with a plan of 589 thousand (103.29% achievement). The goals of providing the population with doctors working in state and municipal medical organizations (41.52 people) have also been exceeded. for 10 thousand people. with a planned annual value of 40.1 (103.54% of achievement)).
According to the Ministry of Health of Russia, the satisfaction of the population with medical care in 2023 reached 47.6% against 41.4% a year earlier.[2]

According to Rosstat estimates, in 2022, approximately 10.6% of the staff of ambulance stations (departments) were doctors, another 56.1% were nurses. For comparison: in 2020, the share of doctors was 11.4%, while the indicator for the average medical staff did not change.
The Rosstat report says that during 2022, ambulance teams served a total of 40,728,350 people. In particular, assistance was provided to 627,352 children. 674,705 people received assistance from aviation medical teams.
It is also said that in 2021, out of 27.3 thousand full-time medical positions at stations and in emergency departments, 18.5 thousand were occupied. They employed 13 thousand specialists. In the segment of nursing staff out of 136 thousand full-time positions, 118.2 thousand were employed; approximately 93.6 thousand people worked for them.[3]

According to HeadHunter, the demand for dental specialties in Russia is growing annually. The number of doctors in Russia in 2022 decreased by 1.3% compared to the previous year and amounted to 37.2 specialists per 10 thousand population against 37.7 in 2021. Such data are given in the report of the Ministry of Health of the Russian Federation.

Mishustin allowed doctors to work as anesthesiologists and surgeons after completing courses
In December 2022, Prime Minister Mikhail Mishustin signed a decree approving the list of medical specialties in which doctors will be able to work after completing professional retraining courses and primary specialized accreditation. The list includes:
- anesthesiologists-resuscitators;
- orthopedic traumatologists;
- surgeons.
As specified in the government decree, accredited doctors with diplomas in the specialties "medical care" or "pediatrics" will be able to work in these specialties. Training in an additional professional program for the selected profile should take at least 1296 hours (9 months). The decree is valid until December 31, 2024.
Thus, in accordance with the government decree, non-core doctors after courses will be able to work as anesthesiologists and surgeons.

The innovation affected the following specialists in the presence of continuous work experience for more than 3 years:
- specialist doctors of the district hospital in rural areas;
- nurses of rural district hospitals, outpatient clinics and feldsher-obstetric centers;
- district therapists and pediatricians, including heads of specialized departments, in territorial sections of city polyclinics, polyclinic departments;
- district nurses of therapeutic and pediatric territorial areas;
- doctors of visiting ambulance teams, nurses of such teams, including those included in the visiting team for sanitary evacuation and for emergency consultation;
- senior doctors and paramedics of ambulance stations;
- paramedics of medical outpatient clinics and feldsher-obstetric centers;
- doctors and nursing staff of rural residential homes for the elderly and disabled;
- doctors and general practitioners of therapeutic and pediatric areas and departments.
Health workers in rural areas and remote areas, in addition to additional leave, as of the end of December 2021, are offered one-time material support under the Zemsky Doctor and Zemsky Paramedic programs in the amount of 0.5-2 million rubles.
The decree will come into force on January 1, 2022. The Ministry of Health of the Russian Federation was instructed to provide an explanation on the application of the adopted resolution.
