Hypertension (high pressure)
Hypertension is a persistent increase in blood pressure (from 140/90 mm Hg and above), which is one of the most common diseases of the cardiovascular system. Increased blood pressure several times increases the risk of developing fatal diseases, such as coronary heart disease, stroke, heart failure.

Hypertension is known as a "silent killer": despite the fact that this disease is often asymptomatic, it can lead to serious disorders of the cardiovascular system.
Hypertension (AH), which, according to data for 2021, affects about 1.2 billion people in the world, is the most common fatal disease. About 10 million people die from high blood pressure in the world every year. According to forecasts, by 2025 this disease will be diagnosed in 29.2% of the adult population of the planet, which is 1.56 billion people.
In Russia, the prevalence of arterial hypertension, as of 2019, is 44.4% of the total population, that is, 65,170,639 people, and this figure is expected to [1]

The share of Russians with high blood pressure since 2019 has grown from 22% to 29.9 %
The number of Russians with diagnosed hypertension has increased significantly over the past five years. As of October 22, 2024, the proportion of identified patients with high blood pressure reached 29.9% of the total number of examined, while in 2019 this figure was 22%.
According to Kommersant, these statistics were announced by Yevgeny Shlyakhto, Director General of the V. Almazov National Medical Research Center, at a round table in the State Duma. In Russia, 32% of the adult population suffer from high blood pressure.
SinceThe head of the State Duma Health Protection Committee Badma Bashankaev stressed that hypertension annually leads to 10 million premature deaths and about 200 million cases of disability in the world. Elevated pressure is a major risk factor for coronary heart disease, heart attacks and strokes.
Ramil Khabriev, scientific director of the Semashko National Research Institute of Public Health, cited data on the previously existing program to combat hypertension. Since 2001, the program has reduced the incidence of cerebrovascular diseases and coronary heart disease by 13-18%, disability - by 10-12%, mortality from strokes and heart attacks - by 10-16%. The prevented economic damage amounted to ₽22,4 billion.
Advisor to the Minister of Finance Vladimir Shalaev said that federal budget expenditures on drug supply in the outpatient sector by 2024 increased to ₽387,4 billion. In 2025, it is planned to allocate ₽435 billion for these purposes, in 2026 - ₽469,1 billion, in 2027 - ₽494,5 billion.
Tatyana Frolova, a member of the Duma Health Protection Committee, noted that patients with hypertension who have not suffered a heart attack or stroke should be provided with medicines at the expense of regional budgets, but the subjects do not have enough funds for these purposes.[2]

European Community of Cardiology recommends kidney denervation to treat blood pressure
At the end of February 2023, it became known that patients whose high blood pressure remains despite taking drugs and changing their lifestyle can be treated with renal denervation. The relevant recommendation was made by the European Community of Cardiology.
Renal denervation is a minimally invasive procedure that uses a radiofrequency or ultrasound device on a catheter to remove nerves in the renal artery. Early in 2022, the threat of failed clinical trials loomed over the procedure, but results from later randomized controlled trials point to safety.
Based on these results, a panel of EU experts recommended renal denervation, as a treatment method as an alternative for patients whose high blood pressure does not lend itself to lifestyle changes and taking a combination of three drugs for pressure. This is also an acceptable option for patients who cannot tolerate long-term therapy with antihypertensive drugs. According to a consensus statement, it can be used as an earlier intervention if patients express a strong preference for kidney denervation after intensive consultation with a doctor.
In a review of the results, European experts noted no particular safety concerns other than radiation and a potential complication associated with an artery access procedure that occurred in less than 1% of cases. In the long term, patients may develop artery stenosis associated with vascular damage and impaired kidney function, according to the study, the results of which were published in the European Heart Journal.
A consensus statement should support the insurance coverage of this procedure in Europe. Medtronic's Simplicity Spyral renal denervation system has had a CE Mark since 2013, but insurance coverage has been problematic as it has been recommended in the past for use only in clinical trial settings. In the US, Medtronic submitted the results of the SPYRAL HTN-ON MED study to the FDA, hoping to win approval in 2023.[3]

Increase in the number of new cases of hypertension by 57%

At the same time, about 42% of men and about 62% of women take drugs to reduce blood pressure, so the task of identifying patients with hypertension is one of the most important in the framework of medical examination and preventive medical examinations of the adult population, he added.
In Russia, until 2024, the national project "Healthcare" will be implemented, on which it is planned to spend more than 1.7 trillion rubles. The project, in particular, involves work to reduce mortality from cardiovascular diseases and re-equipping 140 vascular centers and 469 primary vascular departments with modern medical equipment.[4]
