Brain stroke

Pirogov of the Ministry of Health of Russia, Ph.D. Alexander Rozanov, the main provisions of the recommendations for the secondary prevention of stroke include:
- healthy lifestyle, including proper nutrition and motor activity;
- adherence to the low-salt and Mediterranean diet;
- physical rehabilitation under strict medical supervision;
- change in the patient's lifestyle.
"In our country, adherence to a healthy lifestyle is high only in 21.9% of the population, low - in 43.6%. What is the low commitment? More than 50% of the population consume too much salt, 37.5% eat little vegetables and fruits, 22.7% smoke, 19.6% are sedentary and 4.9% drink alcohol excessively, "said Alexander Rozanov.
The specialist recommended that stroke patients follow a diet containing monosaturated fats, vegetable products and fish, with the addition of extra virgin olive oil or nuts, reduced body weight and blood pressure, and control of table salt intake (less than 1.5 g/day).
Patients who have suffered any stroke need to constantly monitor blood pressure, lipid and glucose levels, and use drug prophylaxis. Antithrombotic therapy, including antiplatelet or anticoagulant, is recommended for almost all patients without contraindications.

Scientists cite smoking, non-compliance with dietary rules, kidney problems and obesity, high blood pressure, high cholesterol and sugar as the reason for the increase in deaths from stroke.
The journal The Lancet Neurology (the leading journal in clinical neurology) published a study that reported 101 million cases of stroke between 1990 and 2019 - a figure the study calls the "prevalence" of the disease. In 2019, the age-standardized mortality rate was 3.6 times higher in low-income countries than in high-income countries. Low-, middle-low-income and upper-middle-income states accounted for 86% of all stroke-related deaths and 89% of GFID.

In 2021, an estimated 93.8 million people around the world suffered a stroke, and the number of deaths associated with this ailment amounted to 7.3 million. Stroke was the third most common cause of death after coronary artery disease and coronavirus infection COVID-19. Ischemic stroke was most often diagnosed: it accounted for 65.3% of all new cases in 2021. This is followed by intracerebral hemorrhage (28.8%) and subarachnoid hemorrhage (5.8%).
The burden of stroke varies widely between regions. So, in Luxembourg in 2021, the lowest incidence was noted - 57.7 cases per 100,000 population, and in the Solomon Islands - the highest, amounting to 355 diagnoses per 100,000 inhabitants. Singapore has the lowest death rate from stroke at 14.2 cases per 100,000 population, while North Macedonia recorded the highest at 277.4 deaths per 100,000 inhabitants. Significant differences are observed between high-income and low-income regions, with Central Asia, East Asia and sub-Saharan Africa facing the highest stroke burden. In contrast, high-income regions such as North America and Australasia showed the lowest levels.
In Russia, the number of new cases of stroke in 2021 amounted to 422 thousand, which is 27% less than in 1990. The number of deaths from stroke decreased over 30 years by 43.9% - to 311 thousand. The prevalence of strokes was 2.45 million cases, falling by 9.5%.[5]

You may be familiar with some of the most common lifestyle and health factors that contribute to stroke risk, including smoking, high blood pressure, high cholesterol, and lack of physical activity. However, there is another overlooked factor that could also affect stroke risk - altitude.
Higher altitude means less oxygen availability, so people living on higher ground have adapted to these conditions. However, how this environment affects the likelihood of developing a stroke remains to be seen. Evidence suggests that short-term exposure to low oxygen may contribute to increased blood clotting and risk of stroke, but the risk among people permanently living at high altitude is unclear. Researchers in Ecuador are in a unique position to study these phenomena, as the presence of the Ecuadorian Andes means that people in the country live at a wide variety of altitudes.
The main motivation for our work was to draw attention to an issue that is very little understood. That is, more than 160 million people live at an altitude of more than 2.5 meters, and there is very little information about epidemiological differences in terms of stroke at an altitude. We wanted to contribute to new knowledge in this population, which is often considered the same as the population living at sea level, and from a physiological point of view we are very different, "explained Professor Esteban Ortiz-Prado from the University of Las Americas in Ecuador, lead author of the study. |
Researchers examined hospital records in Ecuador for the period from 2001 to 2017 and estimated the rate of hospitalization and mortality from stroke among people living at four different heights: low (less than 1.5 thousand meters), moderate (1.5-2.5 thousand meters), high (2.5-3.5 thousand meters) and very high (3.5-5.5 thousand meters).
The results showed that people living at higher altitudes (above 2.5 meters) tended to experience stroke later in life compared to those living at lower altitudes. Interestingly, people living at higher altitudes were also less likely to go to hospital or die of a stroke. However, this protective effect was stronger in the range from 2.0 to 3.5 thousand meters and slightly weakened above 3.5 thousand meters.
Perhaps people living at high altitude have adapted to low oxygen conditions and are building up new blood vessels faster to help overcome stroke-induced damage, the researchers clarify. They may also have a more advanced vasculature in the brain that helps them make the most of the oxygen they consume, but it may also protect them from the worst effects of stroke. More research is needed to identify the mechanisms behind the phenomenon, but the findings may provide some comfort to those living on top of the world.[6]
