How ambulance and emergency services are arranged

Who exactly to call - an ambulance or an ambulance, the dispatcher decides: the one who answers the call by phone "03."

The dispatcher works according to the algorithm: first of all, he finds out if there is a threat to life, enters the answer into a program that helps determine the type of help. Questions are highlighted on the screen: what hurts, how it hurts, burns, presses or stabs, where the pain gives. Based on the answers received, the program forms a reason for calling.

Scheme for 2013

Ambulance rules

The dispatcher calls an ambulance team if an accident has occurred, in emergency situations, in case of fractures, wounds, heart attacks, acute poisoning.

If a patient who does not have a policy does not receive help in polyclinics, then ambulance teams cannot refuse anyone - neither people without documents, nor citizens of other countries, nor those who do not have a policy.

According to the regulations, the ambulance must come primarily to public places, to pregnant women and children. In the second - to the sick home. In the third - to patients in medical institutions. It often seems logical to go with your problem to the nearest hospital, but based on the regulations, it turns out that much faster help will come if you go outside and call an ambulance.

If you are refused hospitalization or have other difficulties, you can contact the dispatchers of the substation to which the ambulance team is assigned. The operator should listen to you and either convince the doctors or send another team if you really need to be hospitalized.

You can report a violation of rights and receive legal advice by calling the hotline of the Federal Service for Supervision of Healthcare in Moscow and the region: 8 (495) 611 47 74

You can call Puchkova station at 8 (499) 445 02 63.

Emergency rules

The emergency does not hospitalize, but provides medical care at home, attached to the clinic, and not to the hospital. She comes to the patient if his diagnosis does not pose a threat to life:

  • with influenza or ARVI,
  • complaints of dizziness,
  • headaches and
  • chest pain,
  • difficulty breathing (except asthma),
  • various chronic diseases.

A doctor from the clinic must come to the patient within 24 hours and monitor his condition. This is also a doctor who may limit himself to consulting, or may call one of the medical teams.

Equipping all ambulances with defibrillators-monitors

At the end of November 2023, the Moscow mayor's office told about the upcoming equipping of all ambulances in the city with defibrillators-monitors.

File:Aquote1.png
By the beginning of next year, we plan to equip the entire fleet with new modern defibrillators-monitors. Of the 1200 such devices purchased now, almost 900 are already used by doctors. Thanks to the new monitors, Moscow doctors will be able to provide assistance to patients in emergency situations even faster and more efficiently, which is the primary task of the ambulance, "said Anastasia Rakova, deputy mayor of the capital for social development, whose words are quoted on the portal of the mayor and the government of Moscow on November 28, 2023.

The Moscow mayor's office told about the upcoming equipping of all ambulances in the city with defibrillators-monitors

It also notes that new monitor defibrillators can remotely transmit clinical data about patients and work both manually and automatically - the device itself will check the heart rate and determine the need for defibrillation, as well as adjust the characteristics of the required discharge.

Another function of the equipment installed cars in the ambulance in Moscow is to conduct capnometry, that is, to measure the concentration of carbon dioxide in the air exhaled by the patient. As noted in the mayor's office, this helps to effectively monitor the patient's condition and timely determine the moment when the independent work of the heart is launched, when resuscitation measures can already be stopped. New defibrillator monitors help doctors stabilize the patient in place for arrhythmia, sepsis, severe dehydration, myocardial infarction and many other critical conditions.

New defibrillators-monitors are installed in cars of the A.S. Puchkov Station for Emergency and Emergency Medical Care, in which teams make more than 12 thousand trips per day (data for November 2023).[1]

Conduct approximately 4,000 ECGs daily

About 4,000 ECG studies are carried out by doctors and paramedics of the metropolitan ambulance daily. The transfer of results and their further interpretation takes place in digital format, which allows for high-quality diagnostics and prompt start of patient treatment as soon as possible. This was announced on August 10, 2023 by the chief physician of the Emergency and Emergency Medical Care Station named after A.S. Puchkov Nikolai Plavunov.

File:Aquote1.png
Every day, paramedic ambulance teams are transferred to a cardiological advisory post of about 2,500 ECG studies. The results are transmitted and interpreted in digital format, moreover, the "access points" service is implemented in the mobile tablets of the teams, which allows you to refuse to use additional devices for remote transmission of ECG research. In addition, about 1,500 ECG studies are carried out daily by medical ambulance teams, - said Nikolai Plavunov.

He also added that the medical teams of the Moscow ambulance can independently decipher even the most complex electrocardiograms, and paramedics may need a consultation with a narrow-profile specialist. Thus, the team has additional data to assess the patient's condition and make an accurate diagnosis. If earlier doctors had to consult a cardiologist on the phone and lose time, now this process passes quickly and effectively.

Moscow ambulance fleet updated by 30%

The fleet of the A.S. Puchkov Ambulance and Emergency Medical Care Station for the year has been updated by 30%. On March 19, 2021, the chief physician of the institution, Nikolai Plavunov, spoke about this, summing up the results of his work for the year of the COVID-19 coronavirus pandemic.

According to him, more than 300 new ambulances were purchased - all-wheel drive models Ford, Mercedes and GAZ, equipped with modern equipment to assist in emergency and severe situations, including traumatic brain injuries, severe injuries in road accidents, strokes and heart attacks.

In addition, the Moscow government has purchased portable ultrasound devices for general-profile and resuscitation ambulance teams, thanks to which ambulance teams can conduct ultrasound diagnostics in patients in critical states right at the place of call. The Moscow ambulance was the first in the country to develop and patent survey algorithms using artificial intelligence, Plavunov noted.

The COVID-19 pandemic has become a difficult stage for all of us: both for residents of the capital and, of course, for the entire health care system. But, despite all the difficulties, she brought positive changes. In particular, quite large-scale projects were implemented at the Moscow ambulance, "he said (quoted by TASS).

The head physician of the Ambulance and Emergency Medical Care Station named after A.S. Puchkova also said that cars portable capnographs appeared in the ambulance - with their help, in real time, you can determine the level of carbon dioxide in the air that the patient exhales. This gives doctors the opportunity to perform more effective diagnosis and treatment of conditions associated with impaired breathing.

Also, resuscitation teams were equipped with portable laboratory express analyzers. They make it possible to quickly conduct a biochemical blood test in patients in critical condition already in an ambulance.[2]

Purchase of 70 portable ventilators

On February 10, 2021, it became known about the purchase of 70 portable artificial lung ventilation devices (IVL) for the A.S. Puchkov Station for Emergency and Emergency Medical Care in Moscow. The equipment provides respiratory support to patients in serious condition and even children with critically low body weight, including premature babies. Read more here.

Start using laboratory express blood analyzers in Moscow reanimobiles

At the end of January 2021, the Moscow mayor's office purchased 70 portable laboratory express analyzers, which will be used in automotive assistance.

File:Aquote1.png
Now resuscitation teams will be able to carry out a full range of necessary emergency diagnostic measures as soon as possible. The importance of this device is difficult to overestimate, since it is necessary to save the lives of the most difficult patients when the bill goes for minutes, "said Anastasia Rakova, Deputy Mayor of Moscow for Social Development, whose words are quoted on the official portal of the capital's mayor.
File:Aquote2.png

According to her, all portable laboratory express analyzers will be combined into a single information system in order to be involved in the interpretation of the data of the second expert in case of a non-standard clinical situation.

In the reanimobiles of Moscow, it is now possible to conduct a biochemical blood test

A portable laboratory express analyzer for conducting a biochemical blood test is a small device where a patient's blood sample cartridge is inserted. The main indicators of the blood test appear on the display in a few minutes, the data can also be printed. The device will help in the diagnosis of sepsis, severe renal and acute respiratory failure, disorders of the acid-base state of the blood and to identify the causes of blood circulation arrest, the mayor's office said in a statement.

The website of the mayor of Moscow also says that by the end of January 2021, the A.S. Puchkova is the largest medical organization in the city. It includes 60 substations and 107 posts, of which 20 posts are located on large highways, 77 - on the territory of hospitals and polyclinic medical organizations and 10 - on the Moscow Ring Road. The station employs 2.5 thousand doctors, 8.4 thousand specialists with secondary medical education.[3]

2014: Organization of specialized emergency departments at hospitals

On January 1, 2014, the Minister of Health Veronika Skvortsova introduced specialized emergency departments at hospitals, which receive both patients delivered by ambulance and those who independently sought medical care. Doctors separate patients by the severity of diseases and either prescribe outpatient treatment, or send them to a specialized hospital[4].

2013: Transfer from the federal budget to the local budget, reduction in funding

The key for the work of the Moscow ambulance were changes in the financing of compulsory health insurance: since January 2013, it has been transferred from the federal budget to the local one. As soon as the costs weighed down the city budget, the Moscow authorities decided to cut them. The number of ambulance posts and hospital places is decreasing, and the main burden is taken over by the emergency service revived in 2011.

From March 1, 2013, insurance companies began checking the quality of ambulance work, after which fines were introduced for doctors. Now money from premiums can be withheld for incorrect paperwork or being late.

2012: Rental of ambulances for 6.3 billion rubles

The Moscow ambulance does not have its own fleet, but takes cars for rent with drivers. In December 2012, the Puchkov Ambulance and Emergency Medical Station on the public procurement website placed an order for the rental of 206 ambulances along with drivers, maintenance and fuel. The contract value is 6.3 billion. rubles The lessor turned out to be the State Unitary Enterprise Mosavtosantrans, an enterprise also controlled by the Department health care Moscow and serving mainly the Puchkov Station. That is, the department rents cars at its own price from 400 to 700 rubles per hour.

2011: Revival of Emergency Care

In 2011, the emergency service was revived in Moscow, the main task of which is home treatment, and not hospitalization.
