Opening a new clinic in Kurgan for the treatment of large joints

In October 2024 Barrow , a new medical institution for the treatment of large joints was opened in. The new Clinic for Reconstructive Surgery of Large Joints, organized on the basis, Academician G.A. Ilizarov Scientific and Medical Center for Traumatology and Orthopedics Ministry of Health Russia was created as part of the center's development strategy until 2030. Start implantation of 3D-printed talus prostheses

The G.A. Ilizarov National Medical Research Center for Traumatology and Orthopedics in Kurgan in October 2024 successfully performed an operation to replace the talus and ankle joint with a prosthesis made using 3D printing. This is the fifth such operation performed in the center, and the first in Russia in the treatment of osteomyelitis of the ankle joint.

The patient was a 68-year-old resident of the Tyumen region, who suffered from joint pain and gait disturbances after an injury. Conservative treatment failed, and the woman went to the Center's Bone and Joint Infection Clinic with a diagnosis of aseptic necrosis of the talus.

Implant created on a 3D printer

The operation on total arthroplasty of the left ankle joint was performed by the candidate of medical sciences Anatoly Sudnitsyn. He replaced the damaged joint with a prosthesis and an individually manufactured talus implant. The surgeon noted that the patient had completely destroyed the articulated part of the talus and the distal articular end of the tibia.

Before the operation, a bacteriological study of the contents of the ankle joint was carried out, the negative result of which allowed the use of non-standard treatment tactics. Based on a 3D model of a healthy leg, the patient made an implant adapted for the injured leg.

The use of an individual prosthesis avoided complex two-stage treatment and performed one reconstructive intervention on the talus and ankle joint. The endoprosthesis was installed under the control of X-ray navigation for the most correct implantation.

The postoperative period in the patient proceeds without complications, the wound heals. The use of individually manufactured endoprostheses in other patients of the Clinic for Bone and Joint Infection also showed positive long-term treatment results.[1]

2023: Artificial Intelligence Anesthesia Equipment Begins

On March 9, 2023, the National Medical Research Center for Traumatology and Orthopedics named after Academician G.A. Ilizarov of the Ministry of Health of the Russian Federation announced the start of using an anesthesiological system with artificial intelligence.

Smart anesthesia equipment, as noted, greatly facilitates the work of an anesthesiologist during complex operations for adults and children from birth. The device provides effective feedback, allowing you to constantly diagnose the changing state of the patient. The anesthesia and respiratory system is controlled through a 15-inch touchscreen display. The novelty combines the capabilities of expert monitoring, as well as advanced technologies and means of ventilation.

During operation, the complex correlates patient ventilation depending on the diameter of the endotracheal tube. It is possible to choose a mode of low-flow anesthesia with economical consumption of anesthetics, and constant monitoring of gas flows helps to maintain a stable concentration of inhaled oxygen.

The device itself diagnoses the patient's condition and as soon as some changes occur, immediately signals this to the doctor - it informs us about the change in the state of airway patency, sputum viscosity and also recommends switching to some other mode of operation, - notes the head of the department of anesthesiology and resuscitation of the Ilizarov Center Yevgeny Kochegarov.

In addition, the medical institution received devices for finding nerve plexuses for conducting conductor anesthesia. These devices can be compared with an anatomical atlas: they independently lay not only the trajectory of the introduction ULTRASONOGRAPHY of a positive needle, but also inform what the angle of its tilt should be. Such tools improve the level of safety for the patient during anesthetic procedures.[2]

Installation of a 3D talus implant in a patient with osteomyelitis, for the first time in Russia

As it became known in March 2022, the doctors of the Clinic of Purulent Osteology of the Ilizarov Center installed a titanium implant of the talus of the ankle joint manufactured by FSUE "CITO" using 3D technologies. Such an operation was carried out in Russia for the first time.

Specialists of FSUE "CITO" together with doctors of the Clinic of Purulent Osteology of the Ilizarov Center designed and manufactured using 3D technologies an implant of the talus bone of the ankle joint, which was installed to a 66-year-old man with osteomyelitis (purulent bone damage). Previously, patients with such a diagnosis either had their foot amputated or had their affected joint completely immobilized with a shortened limb.

The first patient who managed to avoid amputation was a 66-year-old native of Krasnoufimsk, Sverdlovsk Region, who received an open ankle injury. Later, a complication was added - osteomyelitis.

The doctors of the Ilizarov Center removed the rotted talus bone to the patient. Then they put a temporary cement prosthesis, which had the ability to antibacterial protection and suppress infection.

And already at the third stage, an individual titanium prosthesis was installed, which completely repeats the relief of the removed bone. This prosthesis was made on the basis of multispiral computed tomography with 3D reconstruction and printed on a special printer.

The implant manufactured by FSUE "CITO" made it possible to completely replace the functions of the joint and reduce the recovery time. A week later, the patient was able to engage in physical therapy and gradually increase the load on the operated leg.[5]

As the Ilizarov center emphasized, similar high-tech assistance for the patient is absolutely free, it is carried out at the expense of the state either by quota or by the compulsory medical insurance policy.

More than 12 thousand people undergo treatment and rehabilitation in the clinic annually.

2017: Creating a verticalizer for paralyzed people

In July 2017, it became known about the development of a verticalizer for paralyzed people at the Russian Scientific Center named after G. A. Ilizarov. The device allows lifting a paralyzed person to an upright position and moving around an apartment or rough terrain.

As the head of the center, Alexander Gubin, told TASS at the Innoprom-2017 international industrial exhibition, the verticalizer is an all-wheel drive chair machine capable of lifting a paralyzed patient and bringing him to the necessary object.

The device is designed to prevent and eliminate complications associated with the patient's long stay in a lying position.

By mid-July 2017, the new development reached the stage of readiness for serial production. The scientific center is looking for partners with the help of which it will be possible to establish a mass production of the product.

In addition to the verticalizer for paralyzed people, the Russian Scientific Center named after G. A. Ilizarov created prototypes of Ilizarov's automatic compression and distraction medical apparatus, which accelerates the process of bone tissue regeneration. To do this, special micromotors are installed that independently stretch the bone. After installing the device, the patient can leave the clinic.[6]
